布地奈德联合肺表面活性物质预防早产儿支气管肺发育不良疗效观察

摘　　要：目的 探讨布地奈德（BUD）联合肺表面活性物质（PS）对呼吸窘迫综合征（NRDS）早产儿支气管肺发育不良（BPD）的影响.方法 2016年8月至2017年8月新生儿科呼吸窘迫综合征（NRDS）患儿150例,随机分为2组,各75例.对照组给予PS治疗,观察组给予BUD＋PS治疗,比较两组BPD发生率、吸气时间、血气等指标.结果 用药前两组患儿血气指标比较,差异无统计学意义（P〉0.05）;与用药前比较,用药12h、24h、48h后两组各血气指标均明显好转（P〈0.05）;且观察组优于对照组（P〈0.05）.两组BPD发生率、住院天数比较差异有统计学意义（P〈0.05）.两组PIP、PEEP、RR等指标比较差异有统计学意义（P〈0.05）.结论 BUD联合PS治疗NRDS,可有效降低BPD发生率,改善血气指标,促进恢复,缩短住院时间及吸氧时间,具有较高的应用价值.Objective To explore the effect of the budesonide （ BUD ） combined with pulmonary surfactant （ PS ） for preventing the bronchial puhnonary dysplasia （ BPD ） . Methods 150 cases of neonatal respiratory distress syndrome （ NP, DS ） from August 2016 to August 2017 during our new pediatric treatment were randomly divided to group A （ n=75, PS ） and group B （ n=75, BUD ＋ PS ） The blood gas indexes of two groups of children were observed and the incidence of BPD, total oxygen time, etc were compared. Result The blood gas indexes of the two groups were compared before treatment, the difference was not statistically significant （ P〉0.05 ） . Compared with before treatment, blood gas indexes of the two groups were significantly improved after 12h, 24h, and 48h （ P〈0.05 ） , and the observation group was superior to the control group （ P〈0.05 ） . In the two groups, the incidence of BPD breathing, machine use indicators such as time, suction time, length of hospital stay were compared with statistical significance （ P〈0.05 ） . Two groups of PIP, PEEP and RR breathing machine indexes were compared with statistical significance （ P〈0.05 ） . Conclusion BUD joint PS applied in NRDS, can effectively reduce the incidence ofBPD, improve blood gas index, promote recovery, shorter hospitalization time and oxygen, has higher application value.
