某三级医院铜绿假单胞菌临床分布及耐药性研究

摘　　要：目的了解临床分离出的铜绿假单胞菌(PA)的分布及其耐药性,为临床合理选用抗菌药物提供参考。方法回顾性分析某大学附属医院住院患者送检标本中培养出的PA的临床分布及耐药特点并进行总结分析。结果 2013-2015年共分离出PA 439株,主要来源于痰液、咽喉、尿液及脓液,分别占79.17%、8.20%、6.61%和4.78%;PA的检出主要分布于呼吸内科、综合I科、神经内科和泌尿外科,分别占27.11%、23.69%、10.71%和9.11%;2013-2015年,PA对氨曲南和头孢他啶耐药率呈增加趋势,对哌拉西林他唑巴坦耐药率先降后升,对头孢哌酮舒巴坦耐药率呈先升后降趋势,对其他常用抗PA抗菌药耐药率无明显变化。结论 PA的检出来源主要为痰标本,临床主要分布于呼吸内科,PA对部分临床使用强度较高的抗PA抗菌药物耐药率有增加趋势,且耐药率的升高滞后于使用强度的增加。Objective To understand the distribution and drug resistance of clinically isolated Pseudomonas aeruginosa（ PA）,so as to provide references for clinical rational use of antibiotics. Methods The clinical distribution and drug resistance characteristics of PA in a University Affiliated Hospital were analyzed retrospectively. Results Total of 439 PA strains were isolated mainly from sputum,throat,urine and pus in 2013-2015 years,which accounted for 79. 17% ,8. 20% ,6.61% and 4. 78% ,respectively. The detection of PA was mainly in respiratory medicine,comprehensive I,neurology and Department of Urology,accounting for 27. 11% ,23. 69% ,10. 71% and 9. 11% ,respectively,in 2013-2015 year,the resistance rate of PA to Amamethanone and Ceftazidime increased,to piperacillin tazobactam was the first descend then rose,and the resistance rate of Cefoperazone sulbactam first rose and then decreased,there was no significant change in the drug resistance rate of other commonly used anti PA antiseptic drugs. Conclusions The main source of PA is sputum specimen,which is mainly distributed in the respiratory medicine department. The resistance rate of anti PA antibiotics with high intensity of PA is increasing,and the increase of resistance rate is lagging behind the increase of use intensity.
