银杏叶软胶囊联合激素治疗神经性耳鸣临床观察

摘　　要：目的:观察银杏叶软胶囊联合激素治疗神经性耳鸣气滞血瘀证的疗效。方法:选取102例神经性耳鸣气滞血瘀证患者,随机分为治疗组和对照组各51例。对照组行中耳灌注适量地塞米松注射液,治疗组在对照组基础上口服银杏叶软胶囊治疗。2组均连续治疗4周。比较2组患者治疗前后的耳鸣程度评分、耳鸣响度、听阈值、不同切变率全血黏度水平与治疗后的临床疗效。结果:治疗后,治疗组总有效率为94.12%,高于对照组(78.43%),差异有统计学意义(P<0.05)。2组耳鸣程度评分、耳鸣响度,2 kHz、4 kHz的听阈值,115/s、5/s下的全血黏度均较治疗前降低(P<0.01);治疗组以上指标值均低于对照组(P<0.01)。结论:银杏叶软胶囊联合激素治疗神经性耳鸣气滞血瘀证疗效显著,药物可能通过增加患者的耳部组织血供,改善微循环而发挥治疗作用。Objective：To observe the clinical effect of Yinxingye soft capsules combined with hormone for nervous tinnitus with qi-stagnation and blood-stasis syndrome. Methods： Selected 102 cases of patients with nervous tinnitus with qi-stagnation and blood-stasis syndrome,and divided them into the treatment group and the control group randomly, 51 cases in each group. The control group received dexamethasone injection in an appropriate amount by means of perfusing middle ear for treatment,while the treatment group additionally received oral application of Yinxingye soft capsules. The two groups received the treatment for 4 weeks. Compared tinnitus score,tinnitus loudness,hearing threshold,and whole blood viscosity at different shear rates before and after treatment as well as the clinical effect after treatment of both groups.Results：After treatment,the total effective rate was 94.12% in the treatment group,being higher than that in the control group（78.43%）,difference being significant（P〈0.05）. Tinnitus scores,tinnitus loudness,hearing threshold at 2 kHz and 4 kHz,and whole blood viscosity at 115/s and 5/s in the two groups were lower than those before treatment（P〈0.01）. These previous indexes in the treatment group were all lower than those in the control group（P〈0.01）. Conclusion：The therapy of Yinxingye soft capsules combined with hormone has a significant clinical effect in treating nervous tinnitus with qi-stagnation and blood-stasis syndrome, and the drugs may increase blood supply of ear tissue of the patients, so as to improve microcirculation thus to play a therapeutic role.
