摘  要:目的:观察自拟中药治疗脾虚痰湿型非酒精性脂肪性肝炎疗效,并且了解脂肪肝时内毒素血症影响肿瘤坏死因子等炎性介质高表达、胰岛素抵抗,导致氧应激/脂质过氧化和炎症损伤表现。方法:选择脾虚痰湿型非酒精性脂肪肝患者95例,随机分为两组,治疗组63例,对照组32例,两组患者均口服甘草酸二铵150mg/次,3次/d,治疗组患者加用自拟中药颗粒剂,两组均治疗3个月。结果:自拟中药颗粒剂治疗非酒精性脂肪性肝炎可以降低患者高胰岛素状态,改善患者胰岛素抵抗、高脂血症和脂质过氧化指标,降低炎性因子水平,改善肝功能,抗肝纤维化等。结论:脂肪肝患者辩证应用中药是安全有效的治疗方法。Objective: To observe the efficacy of self-made Chinese medicines in the treatment of non-alcoholic steatohepatitis of the Pixu Tanshi type,and to explore the effects of endotoxemia on high expression of inflammatory mediators such as tumor necrosis factor and insulin resistance generate oxidative stress/lipid peroxidation and inflammatory injury in fatty liver. Methods: 95 Pixu Tanshi type non-alcoholic fatty liver patients were randomly divided into treatment group( 63 cases) and control group( 32 cases). Both treatment and control group were given diammonium glycyrrhizinate 150 mg,tid. The treatment group was treated with self-made herbal particles agent meanwhile. Treatment time was 3 months in both groups. Results: Self-made herbal granules could reduce high insulin status,improve patients' insulin resistance,hyperlipidemia and lipid peroxidation index,reduce inflammatory factor levels,improve liver function,and resist liver fibrosis in process of treating non-alcoholic adipose hepatitis patients. Conclusion: The dialectical and rational application of traditional Chinese medicine in patients with fatty liver is a safe and effective treatment.
