加味四物颗粒对老年高血压早期肾损伤的疗效及血清IMD、CysC、FGF23水平影响

摘　　要：目的：探讨加味四物颗粒对老年高血压早期肾损伤患者的疗效及血清叶素（IMD）、胱抑素（CysC）、成纤维细胞生长因子23（FGF23）水平影响。方法：老年高血压早期肾损伤患者104例,采用前瞻性随机研究分为中医辅助组（加味四物颗粒结合基础治疗）、常规治疗组（基础治疗）各52例,疗程8周;对比两组降压效果及相关实验室指标。结果：中医辅助组显效61.54%、有效32.69%、无效5.77%,常规治疗组显效42.31%、有效42.31%、无效15.38%,两组比较差异具有统计学意义（P〈0.05）;治疗后,中医辅助组的SBP、血清IMD、CysC、FGF23及Scr水平均低于常规治疗组（P〈0.05）;治疗后,中医辅助组的眩晕、头痛、腰膝酸软、五心烦热、心悸、失眠、耳鸣、健忘、舌红少苔、脉弦细数积分均低于常规治疗组（P〈0.05）。结论：加味四物颗粒辅助治疗老年高血压早期肾损伤患者有助于降压及改善患者各项中医证候的效果,同时降低血清IMD、CysC、FGF23及Scr水平。Objective：To investigate the influence of Jiawei Siwu Granules on oxytocin（IMD）,serum cystatin（CysC）and fibroblast growth factor 23（FGF23） levels of renal injury elderly patients with hypertension and the early curative effect. Methods：104 elderly hypertensive patients with early renal injury by a prospective randomized study was divided into TCM group（basic therapy combined with Jiawei Siwu Granules） and routine treatment group（basic treatment）,52 cases in each. The treatment lasted for 8 weeks. We compared two groups＇ antihypertensive effect and related laboratory index. Results：TCM group＇s markedly effective rate was 61. 54%,effective rate 32. 69% and invalid rate 5. 77%. The routine treatment group＇s was markedly effective rate was 42. 31%,effective rate 42. 31% and invalid rate 15. 38%.There was statistically significant differences between the two groups（P〈0. 05）. After treatment,TCM group＇s SBP,serum IMD,CysC,FGF23 and Scr were lower than those of the conventional treatment group（P〈0. 05）. The TCM group＇s dizziness and headache,soreness and weakness of waist and knees,dysphoria in chestpalms-soles,palpitation,insomnia,tinnitus,forgetfulness,red tongue,and pulse string count score were lower than those of the conventional treatment group（P〈0. 05）. Conclusion： Jiawei Siwu Granules in adjuvant treatment of elderly hypertensive patients with early renal injury can help lower blood pressure and improve patients＇ TCM syndrome effect,and decrease the levels of IMD,CysC,FGF23 and Scr.
