摘 要:目的探讨丙型肝炎病毒感染与人类白细胞抗原(HLA-DQB1)基因多态性的关系,为寻找丙型肝炎病毒感染的易感基因和拮抗基因提供可参考的科学依据。方法采用PCR-SSP方法对HLA-DQB1等位基因进行检测。比较丙肝患者和对照者之间HLA-DQB1等位基因频率分布差异;进一步分析不同严重程度的丙型肝炎患者HLA-DQB1等位基因频率分布差异。结果丙型肝炎患者的HLA-DQB1*02/03/09基因频率显著高于对照组(P<0.05),HLADDQB1*06基因频率明显低于对照组,差异有统计学意义(P<0.05);肝硬化组HLA-DQB1*02基因频率显著高于无肝硬化组,差异有统计学意义(P<0.05);肝癌组HLA-DQB1*09基因频率显著高于无肝癌组,差异有统计学意义(P<0.05)。结论 HLA-DQB1*02/03/09基因可能是丙型肝炎病毒感染的易感基因;HLAD-DQB1*06基因可能为拮抗基因。HLA-DQB1*02基因可能为肝硬化的易感基因;HLA-DQB1*09基因可能为肝癌的易感基因。本研究为抗病毒治疗或预防HCV的发生提供理论背景。Objective To investigate the relationship between HCV infection and the polymorphism of human leukocyte antigen(HLA-DQB1) gene,so as to provide scientific basis for finding the susceptible genes and antagonistic genes of HCV infection. Methods HLA-DQB1 alleles were detected by PCR-SSP. The difference in the frequency distribution of HLA-DQB1 allele between the patients with hepatitis C and the control was compared,and the difference in the frequency distribution of the HLA-DQB1 allele in the patients with different severity of hepatitis C was further analyzed. Results The frequency of HLA-DQB1* 02/03/09 genes was significantly higher than that of the control group(P 〈 0. 05). The frequency of HLA-DQB1* 06 gene was significantly lower than that of the control group,and the difference was statistically significant(P 〈 0. 05). The frequency of HLA-DQB1* 02 in the liver cirrhosis group was significantly higher than that of the control group,and the difference was statistically significant(P 〈 0. 05). The frequency of HLA-DQB1 * 09 gene in the liver cancer group was significantly higher than that in the non liver cancer group,and the difference was statistically significant(P 〈 0. 05). Conclusion HLA-DQB1* 02/03/09 genes may be a susceptible gene for HCV infection,and HLAD-DQB1 * 06 gene may be an antagonistic gene. HLA-DQB1* 02 gene may be a susceptible gene of cirrhosis,and HLA DQB1 * 09 gene may be a susceptible gene of liver cancer. This study provides a theoretical background for antiviral treatment or prevention of HCV.
