摘  要:目的研究儿童感染金黄色葡萄球菌的耐药现况,比较新生儿MRSA与普通儿童MRSA耐药情况,为临床儿科合理使用抗生素提供依据。方法对2013年-2016年14岁以下儿童标本分离的482株金黄色葡萄球菌进行鉴定和药敏分析,调查不同来源菌株的耐药情况,以及新生儿和普通儿科的分离株进行耐药比较分析。结果从482株菌株中分离到100株MRSA,占20.75%,以呼吸道标本为主(354株,占73.44%),MRSA占18.64%,咽拭子分离MRSA比率高于痰液;新生儿更容易感染MRSA,普通儿科对红霉素和克林霉素耐药率高于新生儿且耐药谱广。结论儿童感染金黄色葡萄球菌以呼吸道为主,特别是咽拭子等上呼吸道标本可疑MRSA定植值得引起注意,新生儿的易感性,耐药谱变宽,使得临床加强耐药监控,控制感染刻不容缓。Objective To study the drug resistance of Staphylococcus aureus in children,and to compare the drug resistance of methicillin-resistant Staphylococcus aureus(MRSA) between normal children and neonates,so as to provide basis for rational use of antibiotics in clinic. Methods Identification and drug sensitivity analysis were performed on 482 strains of Staphylococcus aureus isolated from children under 14 years old from 2013 to 2016. To investigate the drug resistance of Staphylococcus aureus from different sources and analyze the drug resistance of the isolated strains from neonatal and general pediatrics.Results A total of 100 MRSA strains were isolated from 482 strains of Staphylococcus aureus,accounting for 20. 75%,mainly from respiratory specimens(354 strains,73. 44%),MRSA accounted for 18. 64%; the isolation rate of MRSA in throat swab was higher than that in sputum; neonates were more likely to be infected with MRSA. The resistance rates of MRSA to erythromycin and clindamycin in general pediatrics were higher than those in neonates,and the resistance spectrum was wide. Conclusion The children were mainly infected with Staphylococcus aureus through respiratory tract,especially the suspicious MRSA colonization of the upper respiratory tract specimens such as throat swab deserves attention. Neonatal susceptibility and wide drug resistance spectrum make it urgent for the clinic to strengthen drug resistance monitoring and infection control.
