摘 要:目的:通过超声造影时间-强度曲线量化分析阿托伐他汀对颈动脉斑块的影响。方法:86例颈动脉斑块患者,随机分为两组,阿托伐他汀组(每天20mg阿托伐他汀口服)及对照组(口服等剂量安慰剂)各43例,斑块总计125个,随访观察约1年,常规超声观察颈动脉斑块类型,超声造影结合时间-强度曲线观察斑块内造影剂灌注情况,使用声学定量分析软件(SonoLiver)对图像进行脱机分析,计算斑块达峰时间(TTP)、最大峰值强度比(IMAX)、曲线下面积(AUC),并进行组间比较,量化分析阿托伐他汀对颈动脉斑块的影响。结果:治疗前两组各指标差异无统计学意义(P>0.05)。治疗后,阿托伐他汀组颈动脉软斑数量较治疗前减少,斑块内TTP、IMAX、AUC均较治疗前升高或降低,且差异均有统计学意义(P<0.05或P<0.01)。对照组治疗前后各指标差异均无统计学意义(P>0.05)。结论:颈动脉超声造影时间-强度曲线分析结果可为临床采用阿托伐他汀治疗颈动脉斑块提供重要参考价值。Objective: To investigate the influence of atorvastatin on carotid plaques quantitatively analyzed by time-intensity curve of contrast-enhanced ultrasound. Method: 86 cases with carotid plaques were randomly divided into two groups: atorvastatin group (20mg per day oral) and the control group (placebo),each group was 43 cases respectively, a total of 125 plaques were followed for 1 year, the type of which were observed by conventional carotid ultrasound, contrast agent perfusion of carotid plaques were observed by contrast-enhanced ultrasound combined with time-intensity curve, and time to peak(TTP), the ratio of maximum intensity (IMAX)and area under the curve(AUC) of carotid plaques were calculated with analysis software(SonoLiver),which were compared between two groups, and the changes of carotid plaques after atorvastatin intervention also be quantitatively analyzed. Results: There was no significant difference between indexes of the two groups before the treatment (P〉0.05). The number and incidence of carotid soft plaque after treatment was lower than before treatment in atorvastatin group.Compared with the control group after treatment,TTP, the ratio of IMAX, and AUC were significant different.(P〈0.05 or P〈0.01).There was no significant different between control group before treatment and after treatment(P〉0.05). Conclusion: The analysis results of carotid time-intensity curve of contrast-enhanced ultrasound can provide important reference value for clinical treatment of carotid plaques with atorvastatin.
