活动平板心电图运动试验对胸腔手术的术前评估价值

摘　　要：目的:探讨胸腔手术患者术前进行活动平板心电图运动试验的临床应用价值。方法:回顾分析来院就诊并愿意接受胸腔手术且常规心电图正常的患者,观察组412例,对照组450例,活动平板心电图运动试验采用Bruce方案进行。结果:在412例的运动试验患者中,阳性16例,可疑阳性54例,阴性342例。患者心律失常和ST段/T波改变的发生率阳性患者>可疑阳性患者>阴性患者;观察组患者中发生心律失常事件的有49例(11.89%)、ST段或T波改变有36例(8.74%),对照组患者中发生心律失常的有87例(19.33%)、ST段或T波改变有68例(15.11%),观察组患者心律失常和ST段、T波改变的发生率低于对照组(P<0.05)。结论:活动平板心电图运动试验对胸腔手术患者有很好的术前评估价值,值得在外科临床上推广。Objective： To study the clinical preoperative value of treadmill ECG exercise test of the thoracic surgery. Methods： Retrospective analysis was made on patients willing to accept the thoracic surgery and with normal ECG, among whom 412 were in the experimental group and 450 were in the control group. The scheme of Bruce was adopted in the treadmill ECG exercise test. Results： In the 412 patients with the treadmill ECG exercise test, there were 16 cases of positive, 54 cases of suspected positive, and 342 cases of negative. The incidence of cardiac arrhythmias and ST segment or T wave change was： positive〉suspected positive〉negative. In the experimental group, there were 49 cases （11.89 %） of arrhythmia events and 36 cases （8.74 %） of ST segment or T wave change events. In the control group, there were 87 cases （19.33 %） of arrhythmia events and 68 cases （15.11 %） of ST segment or T wave change events. The incidence of cardiac arrhythmias and ST segment or T wave change in the experimental group was significantly lower than that in the control group （ P 〈0.05）. Conclusion： The treadmill ECG exercise test has good estimate value for patient before thoracic surgery, which is worth popularizing in surgical clinic.
