类风湿关节炎患者心外膜脂肪组织厚度与心房功能的相关性

摘　　要：目的探讨类风湿关节炎(RA)患者的心外膜脂肪组织(EAT)厚度及其与心房功能的相关性。方法选取RA患者44例(RA组)及健康成年人30例(对照组)。比较两组超声测量的收缩期EAT(EATs)与舒张期EAT(EATd)厚度,心房收缩期各房壁运动峰值速度(PV)及心房各房壁电-机械收缩时间差,并分析其相关性。结果RA组EATs与EATd均较对照组增厚(P均<0.05)。RA组左房侧壁、后壁、前壁及右房侧壁PV均低于对照组(P均<0.05)。RA组左房侧壁与后间壁、左房前间壁与后壁电-机械收缩时间差均大于对照组(P均<0.05)。RA组EATs厚度与左房侧壁、前壁PV呈负相关(r分别为-0.376,-0.461;P均<0.05);EATs厚度与左房侧壁与后间壁时间差、左房前间壁与后壁电-机械收缩时间差及WHR均呈正相关(r分别为0.640、0.775、0.393,P均<0.05);EATd厚度与左房侧壁与后间壁时间差、左房前间壁与后壁电-机械收缩时间差及WHR均呈正相关(r分别为0.625、0.669、0.334,P均<0.05)。结论 RA患者EATs、EATd均增厚,EAT越厚患者心房功能损害越严重。Objective To measure the epicardial adipose tissue（ EAT） thickness by echocardiography in patients with rheumatoid arthritis（ RA） and to investigate its relationship with atrial function. Methods Forty-four RA patients（ RAgroup） and 30 healthy controls（ control group） were selected. The systolic EAT（ EATs） and diastolic EAT（ EATd） thickness,atrial systolic peak velocity（ PV）,and atrial wall electro-mechanical contraction time difference were compared and their correlations were analyzed. Results The thickness of EATs and EATd increased in the RA group（ P〈0. 05）. In the RA group,the PV of left atrial lateral,posterior,anterior wall and right atrial lateral wall was lower than that of the control group（ P〈0. 05）. Moreover,the electro-mechanical contraction time difference between left atrial lateral and posterior wall,anterior and posterior septum wall was higher than that of the control group（ P〈0. 05）. In the RA group,EATs was negatively correlated with PV of the left atrial lateral and anterior wall（ r =-0. 376,-0. 461; both P〈0. 05）. Both EATs and EATd were positively correlated with the time difference between left atrial lateral and posterior septum wall,electro-mechanical contraction time difference between the left atrial anterior septum and posterior wall,and waist-hip ratio（ r= 0. 640,0. 775,0. 393; r = 0. 625,0. 669,0. 334; all P〈0. 05）. Conclusions EATs and EATd increase in RA patients,the thicker the EATs,the more severe injury of the patients＇ atrial function.
