摘 要:目的探讨新生儿缺氧缺血性脑病(hypoxic-ischemic brain damage,HIE)患儿血清miRNA-21表达及其对新生儿HIE发生发展的意义,为新生儿HIE的早期诊断和预后提供科学依据。方法将2015年3月-2016年11月在新乡医学院第一附属医院出生的新生儿,分为:缺氧缺血组(HIE组)51例并用神经节苷脂对其进行治疗;正常对照组28例,共计79例。两组于生后24~48 h内及治疗15 d后分别采静脉血5 ml。利用实时荧光定量PCR检测血清miRNA-21和低氧诱导因子1α(HIF-1α)的表达水平。结果治疗前HIE组与对照组比较,HIE组血清miRNA-21含量(5.25±2.17)显著高于对照组(1.21±0.10)(P<0.05);血清HIF-1α含量(8.76±3.23)高于对照组(1.28±0.10)(P<0.05)。治疗15 d后,HIE组血清miRNA-21含量(2.13±1.06)和HIF-1α含量(3.64±3.34)与治疗前(5.25±2.17、8.76±3.23)相比,均显著降低(P<0.05)。结论血清miRNA-21和HIF-1α高表达与新生儿缺氧缺血性脑病相关,有望作为新生儿缺氧缺血性脑病早期诊断和预后的生物学指标。Objective To explore the expression of serum microRNA-21( miRNA-21) in neonates with hypoxic-ischemic encephalopathy( HIE) and its significance in the occurrence and development of HIE in neonatal patients,and to provide a scientific basis for the early diagnosis and prognosis of HIE in neonates. Methods Seventy-nine neonates born in the First Affiliated Hospital of Xinxiang Medical College from March 2015 to November 2016 were divided into the HIE group( n = 51) and the normal control group( n = 28). 51 neonatal patients in the HIE group were treated with ganglioside,5 ml of venous blood were collected from each neonate in the two groups within 24-48 hours of birth and after 15 days of therapy. The expression levels of serum miRNA-21 and hypoxia inducible factor-1α( HIF-1 a) were detected by real-time fluorescence quantitative PCR. Results Before the treatment,the serum levels of miRNA-21 and HIF-1α were significantly higher in the HIE group than in the control group((5.25±2.17) vs.(1.21±0.10),(8.76±3.23) vs.(1.28±0.10),both P〈0.001). After 15 days of therapy,the serum levels of miRNA-21 and HIF-1α were significantly declined as compared with those before the treatment(( 2.13±1.06) vs.( 5.25± 2.17),(3.64±3.34)vs.(8.76±3.23),both P〈0.05). Conclusions High expression of serum miRNA-21 and HIF-1α is correlated with HIE in neonates,which may be used as biological indicators for the early diagnosis and prognosis of neonatal HIE.
