摘 要:目的观察曲美他嗪对非体外循环冠状动脉旁路移植术(of-pump coronary artery bypass,OPCAB)患者肌酸激酶(creation kinas,CK)和肌酸激酶同工酶(creation kinas-MB,CK-MB)以及心肌肌钙蛋白I(cardiac troponin I,cTnI)的影响,探讨其心肌保护作用机制。方法将103例于本院择期行OPCAB的冠心病患者随机分为曲美他嗪组和对照组。曲美他嗪组于术前7天服用曲美他嗪20 mg tid,分别于术前、术后6 h、术后12 h和术后24 h抽取静脉血,测定血清CK、CK-MB及c Tn I。结果两组患者临床特征及桥血管情况差异无统计学意义(P<0.01)。曲美他嗪组术前CK、CKMB、c Tn I比较,差异无统计学意义(P<0.01),在术后6、12、24 h明显低于对照组(P<0.01)。结论曲美他嗪可明显降低OPCAB患者CK、CK-MB和c Tn I的释放。Objective To observe the effects of trimetazidine on serum level of the patients, CK、CK-MB and cTnI underwent of-pump coronary by pass, and discusses the mechanism of the protection on myocardium. Methods 103 patients who underwent off-pump coronary artery bypass randomly divided into trimetazidine group(52 cases)and control group(51 cases).To draw vein blood preoperative postoperative 6 hours, postoperative1 2 hours and postoperative 24 hours for analyze CK. CK-MB and cTnI. Results There was no significant difference between 2 groups of clinical characterizes and grafts. There was no significant difference between two groups of serum level of CK before surgery. The serum level of CK of trimetazidine group was lower than control group after surgery 6 hours, 12 hours and 24, respectively (P〈0.01 ). No significant difference between 2 groups of serum level of CK-MB before surgery. The serum level of CK-MB trimetazidine group was lower than control group after surgery six hours, 12 hours and 24 hours,respectively (P〈O.O1). Conclusion Trimetazidine may reduce the release of the serum level of CK.CK-MB and cTnI of the patients underwent OPCAB.
