摘　　要：目的探讨A型肉毒毒素对幼龄脑瘫患儿腓肠肌痉挛的影响。方法收集2014年6月—2016年2月郑州大学第三附属医院收治的94例腓肠肌痉挛的幼龄脑瘫患儿为研究对象,按照治疗方法分为观察组与对照组,对照组给予4个疗程常规康复训练,观察组在对照组基础上注射A型肉毒毒素;比较两组在治疗前、治疗后腓肠肌痉挛的程度、肌张力及粗大运动功能。结果治疗前两组MAS量化评分、腓肠肌s EMG数值及GMFM评分差异无统计学意义(P>0.05),治疗后两组MAS量化评分、腓肠肌s EMG数值较治疗前降低,且观察组低于对照组(P<0.05);GMFM评分较治疗前增高,且观察组高于对照组(P<0.05)。结论 A型肉毒毒素能明显改善幼龄脑瘫患儿腓肠肌痉挛的程度。Objective To investigate effect of Botulinum Toxin A on sural spasm in young children with cerebral palsy. Methods A total of 96 young children with cerebral palsy admitted during June 2014 and February 2016 were divided into observation group and control group. Control group received 4 course of routine rehabilitation training,while control group was added with Botulinum Toxin A injections on the basis of treatment for control group. Degrees of sural spasm,muscle tonus and gross motor function before and after treatment were compared in two groups. Results Before treatment,there were no significant differences in modified Ashworth scales（ MAS） quantitative score,gastrocnemius surface electromyography（ s EMG） values and gross motor function measure（ GMFM） score between two groups（ P〈0. 05）. After treatment,MAS quantitative scores and gastrocnemius s EMG values were significantly lower than those before treatment in two groups,but the scores and values in observation group were significantly lower than those in control group（ P〈0. 05）; GMFM scores were significantly higher than those before treatment in two groups,and the score in observation group was significantly higher than that in control group（ P〈0. 05）. Conclusion Botulinum Toxin A may obviously improve the degree on sural spasm in young children with cerebral palsy.
