责任制助产护理对初产妇产程及分娩结局的影响观察

摘　　要：目的探讨对初产妇实施责任制助产护理对产程及分娩结局的影响。方法选取我院产科2016年6月—2017年6月收治的初产妇90例作为研究对象,随机分成2组,对照组(n=45)实施常规护理,观察组(n=45)采用责任制助产护理,比较2组产程及分娩结局。结果观察组产妇下肢麻木发生率为8.9%,产程异常率为2.2%,产后出血发生率为2.2%,新生儿酸中毒发生率为4.4%,新生儿窒息发生率为2.2%,胎儿窘迫发生率为6.7%,均明显低于对照组(P<0.05);观察组自然分娩率为91.1%,明显高于对照组的68.9%,差异有统计学意义(P<0.05);观察组总产程短于对照组,新生儿Apgar评分高于对照组,产后2 h出血量少于对照组,差异均有统计学意义(P<0.05)。结论责任制助产护理可有效缩短初产妇产程,改善分娩结局,值得推广。Objective The clinical effect of midwifery on the course of childbirth and the outcome of childbirth in primipara. Methods We selected 90 cases of primipara who were admitted to our department from June 2016 to June 2017,they were randomly divided into two groups,the control group（n=45） received routine nursing care,the observation group（n=45）adopted the responsibility system midwifery nursing,the birth course and delivery outcome were compared between the two groups. Results The incidence of neonatal acidosis was 4.4%,the incidence of neonatal asphyxia was 2.2%,and the incidence of fetal distress was significantly higher than that of the control group（P〈0.05）.The incidence of neonatal asphyxia was 2.2%,the incidence of postpartum hemorrhage was 2.2%（P〈0.05）.The rate of natural delivery was 91.1% in the observation group,which was significantly higher than that in the control group（68.9%）,and the difference was statistically significant（P〈0.05）.The total birth rate of the observation group was significantly shorter than that of the control group,the Apgar score of the neonates in the observation group was higher than that in the control group,the bleeding rate was less than that in the control group at 2 hours after operation. The difference was statistically significant（P〈0.05）.Conclusion The responsibility system of midwifery care can effectively shorten the obstetric stage of primipara,It is worth popularizing to improve the outcome of labor.
