摘 要:目的观察溶血对己糖激酶(HK)法测定血清葡萄糖(Glu)的干扰。方法采用美国临床实验室标准化协会(CLSI)EP7-A2文件中的配对差异实验,对干扰物血红蛋白(Hb)进行确认,采用剂量效应实验探讨Hb浓度与干扰程度之间的相关性。结果配对差异实验证实5g/LHb对中、高浓度Glu均存在影响。5个浓度剂量效应实验证实Hb对Glu测定产生非线性正干扰,中浓度Glu的非线性方程为Y=6.496+0.1605X-0.01310X^2(r^2=0.983),高浓度Glu的非线性方程为Y=10.13+0.2110X-0.01783X^2(r^2=0.973)。结论溶血会对HK法测定Glu产生正干扰,实验室应尽量避免样本溶血,若样本无法再次获取,检验报告中应备注溶血程度和可能造成的偏移,为临床提供参考。Objective To evaluate the influence of hemolysis on serum glucose (Glu) determination byhexokinase(HK)assay. Methods The hemoglobin (Hb) was identified as an interfering substance by paireddifferenceexperiment according to the Clinical and Laboratory Standards Institute (CLSI) EP7-A2 document. Thecorrelation between Hb level and interference was evaluated by dose-effect experiment. Results Paired-differenceexperiment showed that Hb at 5 g/L could influence high-level and middle-level Glu determinations. Dose-effectexperiment with 5 levels of Hb showed that there existed non-linear interference in Glu determination. The nonlinearequations were Y=6.496+0.160 5X-0.013 10X2 for middle-level Glu determination (r2=0.983) and Y=10.13+0.211 0X-0.017 83X2 for high-level Glu determination (r2=0.973). Conclusions There exists interference for Gludetermination from hemolytic samples by HK assay. It should avoid hemolytic samples if necessary. If another sampleis not available,the hemolysis degree and potential interference should be attached to the laboratory report sheets forclinical reference.
