风心病二尖瓣狭窄患者左房功能的超声评价

摘　　要：目的:探讨实时三维超声心动图(RT-3DE)对风湿性心脏病单纯二尖瓣狭窄(MS)患者左房功能的评价。方法:应用RT-3DE测量并计算46例风湿性心脏病MS患者及46例正常健康人左房相关指标和指数,对其进行比较。结果:MS组左房最大容积指数(LAVmax I)、左房最小容积指数(LAVmin I)和左心房收缩前容积指数(LAVpre I)高于对照组(P<0.05),而左房扩张指数(LAEI)、左房整体射血分数(LAEF)、左房被动射血分数(LAEFpassive)和左房主动射血分数(LAEFactive)低于对照组(P<0.05)。MS组LAVmax I、LAVmin I、LAVpre I、LAEF和LAEFpassive与二尖瓣口面积(MVA)相关(P<0.05)。结论:风湿性心脏病MS患者左房存储器功能、管道功能、泵功能均降低,RT-3DE可评价风湿性心脏病MS患者左房功能。Objective： To explore the evaluation of left atrial function in patients with rheumatic heart disease and mitral stenosis（MS） by real-time three-dimensional echocardiography（RT-3 DE）. Methods： The left atrial-related indexes in 46 normal healthy people（control group） and 46 patients with rheumatic heart disease and MS（MS group） were measured by RT-3 DE. And then the results were compared between the two groups. Results： The left atrial maximum volume index（LAVmax I）,the left atrial minimum volume index（LAVmin I） and the left atrial pre-systolic volume index（LAVpre I） in the MS group were all higher than those in the control group（P〈0. 05）. However,the left atrial dilatation index（LAEI）,the left atrium holistic ejection fraction（LAEF）,the left atrial passive ejection fraction（LAEFpassive） and the left atrial active ejection fraction（LAEFactive） were all lower than those in the control group（P〈0. 05）. In MS group,LAVmax I,LAVmin I,LAVpre I,LAEF and LAEFpassive were correlated with mitral orifice valve area（MVA）（P〈0. 05）. Conclusion： The left atrial memory function,conduit function and pump function are all decreased in patients with rheumatic heart disease and MS. RT-3 DE can evaluate their left atrial function.
