宫腔灌注中药复方凋膜止崩液治疗子宫内膜增生症大鼠的效果及对子宫内膜相关因子表达的影响

摘　　要：目的探讨宫腔灌注中药复方凋膜止崩液治疗子宫内膜增生症大鼠的效果及对子宫内膜缺氧诱导因子-1α(HIF-1α)和血管内皮生长因子(VEGF)表达的影响。方法将48只SD大鼠随机分为空白组、模型组、大剂量组和小剂量组,每组各12只。除空白组外,其余3组均采用卵巢去势联合雌激素负荷法建立大鼠子宫内膜增生症模型,造模成功后空白组和模型组宫腔灌注生理盐水,大剂量组与小剂量组分别宫腔灌注含生药0.005 g/ml、0.0025 g/ml的凋膜止崩液,3 d后处死动物留取子宫标本,观察大鼠子宫内膜组织学变化,并检测大鼠子宫内膜HIF-1α和VEGF的表达情况。结果镜下观察到各剂量组子宫内膜出现上皮脱失、腺体坏死、血管扩张出血等现象,并有炎细胞的浸润。与空白组比较,模型组与各剂量组大鼠子宫内膜组织中HIF-1α和VEGF表达增高(P<0.05);与模型组比较,各剂量组大鼠子宫内膜中HIF-1α和VEGF表达下降(P<0.05);但大剂量组与小剂量组间差异无统计学意义(P>0.05)。结论宫腔灌注中药复方凋膜止崩液可能通过下调子宫内膜组织HIF-1α和VEGF的表达,从而抑制子宫内膜的持续增生。Objective To explore efficacy of intrauterine infusion of Chinese herbal compound formula Diaomo Zhibeng Decotion for rats with endometrial hyperplasia and its effect on the expressions of hypoxia inducible factor 1α（ HIF-1α） and vascular endothelial growth factor（ VEGF） in endometrium. Methods Forty-eight SD rats were randomly divided into blank group,model group,high-dosage group and low-dosage group,with 12 rats in each group. Endometrial hyperplasia model was established by ovariectomy combined with xenoestrogen burden in the other three groups except the blank group. After the successful modeling,intrauterine infusion of normal saline was conducted in the blank group and model group,intrauterine infusion of Diaomo Zhibeng Decotion was conducted in the high-dosage group and low-dosage group（ the concentrations of crude drug were 0. 005 g/ml and 0. 0025 g/ml respectively）. All rats were killed three days later,then the uteruses were removed. The endometrial histological changes of the rats were observed,and the expression levels of HIF-1α and VEGF in endometrium were detected. Results Epithelial deletion,glandular necrosis,hemangiectasia,hemorrhage and infiltration of inflammatory cells in the endometrium were observed under microscope in the high-dosage group and low-dosage group.Compared with the blank group,the expressions of endometrial HIF-1α and VEGF increased in the rats of the model group,high-dosage group and low-dosage group（ P 〈0. 05）. Compared with the model group,the expressions of endometrial HIF-1α and VEGF decreased in the high-dosage and low-dosage groups（ P〈0. 05）; however,there was no significant difference between the high-dosage and low-dosage groups（ P 〉0. 05）. Conclusion Intrauterine infusion of Chinese herbal compound formula Diaomo Zhibeng Decotion can inhibit the constant endometrial hyperplasia probably by downregulating the expressions of HIF-1α and VEGF in endometrial tissues.
