美罗培南联合万古霉素鞘内注射对开颅术后颅内感染的治疗效果评估

摘　　要：目的分析美罗培南联合万古霉素鞘内注射对开颅术后颅内感染的治疗效果。方法将84例开颅术后颅内感染分为两组,每组42例。对照组静滴万古霉素注射液,观察组接受美罗培南联合万古霉素鞘内注射治疗,2周后,分析结果。结果和对照组相比,观察组治疗效果明显较好,差异具有统计学意义(P<0.05)。观察组治疗后颅内压以及WBC计数明显较低,差异具有统计学意义(P<0.05)。两组治疗后脑脊液内蛋白质以及葡糖值比较差异无统计学意义。治疗后,和对照组相比,观察组的TNF-α、IL-6以及CRP值明显更低,差异具有统计学意义(P<0.05)。两组不良反应比较差异均无统计学意义。结论对于经开颅术后颅内感染者,使用美罗培南联合万古霉素鞘内注射治疗,效果显著,可消除脑脊液内炎性物质,安全性强,全面促进疾病转归,值得推广。Objective To analyze the therapeutic effect of meropein combined with vancomycin intrathecal injection on intracranial infection after craniotomy. Methods 84 cases of intracranial infection were divided into 2 groups with 42 cases. The control group was treated with vancomycin injection, and the observation group was treated with meropein combinedwith vancomycin sheath. After 2 weeks, the results were analyzed.Results Compared with the control group, the therapeutic effect of the observation group was significantly better（P〈0.05）. Observation group after treatment was associated with significantly lower intracranial pressure and the WBC count, the difference was statistically significant（P〈0.05）. In the two groups after treatment of cerebrospinal fluid protein and glucose values have no obvious difference. After treatment, compared with control group, the observation group of TNF alpha, IL-6, and significantly lower CRP values, the difference was statistically significant（P〈0.05）. No obvious difference was found between two groups of adverse reactions. Conclusion For the intracranial infection after craniotomy, use meropenem joint vancomycin intrathecal injection treatment, the effect significantly, can eliminate cerebrospinal fluid inflammatory substances in the body, strong security, comprehensively promote the disease outcome, is worth promoting.
