高频超声在血友病膝关节病变诊断中的应用价值

摘　　要：目的探讨高频超声检查在血友病膝关节病变诊断中的应用价值。方法对19例临床诊断为血友病的患者行膝关节超声检查,并同时行X线检查。分析血友病膝关节病变的超声特点,比较两种影像学方法对膝关节病变检出的一致性。结果 19例血友病患者38个膝关节超声影像表现关节积液55.26%(21/38),滑膜增厚42.10%(16/38),滑膜增厚伴血管增生34.21%(13/38),纤维隔膜15.78%(6/38),软骨改变39.47%(15/38),骨破坏36.84%(14/38),关节间隙变窄28.95%(11/38)。X线检查骨破坏34.21%(13/38),关节间隙变窄34.21%(13/38),两种检查方法对膝关节骨病变检出有很好一致性(c2值分别为3.79,6.74,P<0.05)。结论高频超声检查在血友病膝关节病变诊断中具有重要价值,能为血友病性关节病的病情评估提供重要依据。Objective To investigate the application value of high frequency ultrasound in diagnosis of the disease of the knee joint in patients with hemophilia. Methods 19 patients with hemophilia were examined by ultrasonography, and the X-ray examination was performed simultaneously. To analyze the ultrasonic features of the disease of the knee joint in hemophilia, and compare the consistency between the two imaging methods for the detection of the knee joint lesions. Results 19 cases of patients with hemophilia 38 knee ultrasound imaging features of joint effusion 55.26%（21/38）, synovial thickening respiratary 42.10%（16/38）, synovial thickening with vascular proliferation respiratary 34.21%（13/38）, fiber membrane15.78%（6/38）, cartilage changes 39.47%（15/38）, bone destruction 36.84%（14/38）, joint space narrowing 28.95%（11/38）. X-ray examination of bone destruction 34.21%（13/38）, joint spacenarrowing 34.21%（13/38）, two kinds of examination methods for the detection of the knee joint bone lesions are very good consistency（χ^2=3.79, 6.74, P〈0.05）. Conclusion High frequency ultrasound in the diagnosis of the disease has important value in the diagnosis of the disease,can provide an important basis for the assessment of the disease.
