左归丸治疗老年性骨质疏松症临床观察

摘　　要：目的观察左归丸治疗老年性骨质疏松症(SOP)的临床疗效。方法将60例SOP患者随机分为2组,均予碳酸钙D3片。对照组30例加用左归丸模拟剂治疗;治疗组30例加用左归丸治疗。2组均治疗6个月。比较2组治疗前后血清Ⅰ型胶原交联羧基末端肽(CTX-Ⅰ)、血清Ⅰ型前胶原氨基端前肽(PINP)及β-链蛋白(β-catenin)变化,比较2组治疗前后第1~4腰椎(L1~4)、股骨颈及Ward's三角区的骨密度(BMD),应用简明健康调查问卷(SF-36)评估患者治疗前后生活质量。结果治疗组治疗后CTX-Ⅰ、PINP均较本组治疗前降低(P<0.05),β-catenin升高(P<0.05),且治疗组CTX-Ⅰ、PINP均低于对照组(P<0.05),β-catenin高于对照组(P<0.05)。对照组治疗前后CTX-Ⅰ、PINP及β-catenin比较差异均无统计学意义(P>0.05)。对照组治疗后L1~4、股骨颈及Ward's三角区BMD均较本组治疗前降低(P<0.05),且低于治疗组治疗后(P<0.05)。2组治疗后生理功能、生理职能、躯体疼痛、总体健康、精力、社会职能、情感职能及心理健康8个方面评分均较本组治疗前升高(P<0.05),且治疗组生理功能、生理职能、躯体疼痛、总体健康、精力、社会职能、情感职能及心理健康8个方面评分均高于对照组(P<0.05)。结论左归丸能有效维持SOP患者BMD,降低其骨代谢指标CTX-Ⅰ、PINP,提高血清β-catenin水平,显著改善患者生活质量。Objective To observe the clinical effects of Zuogui-pill on senile osteoporosis（ SOP）. Methods 60 patients with SOP were randomly divided into two groups and all received calcium carbonate D3 tablets. 30 cases in control group were treated by Zuogui-pill simulation,30 cases in treatment group were treated by Zuogui-pill. Both groups were treated for 6 months. The changes of serum type I collagen cross-linked carboxyl terminal peptide（ CTX-I）,serum type I collagen N-terminal propeptide（ PINP） and β-catenin（ β-catenin） were compared between the two groups before and after treatment. The bone mineral density（ BMD） of the lumbar spine（ L1-4）,femoral neck,and Ward＇s trigone were compared between the two groups before and after treatment. The concise health questionnaire（ SF-36） was used to evaluate the quality of life before and after treatment. Results After treatment,CTX-Ⅰ and PINP in treatment group were lower than those before treatment（ P〈0. 05）,and β-catenin was increased（ P〈0. 05）. After treatment,CTX-Ⅰ and PINP were lower in treatment group than in the control group（ P〈0. 05）,and Β-catenin was higher than that in the control group（ P〈0. 05）. There was no significant difference in the levels of CTX-Ⅰ,PINP and β-catenin before and after treatment in the control group（ P〉0. 05）. The scores of physiological function,physiological function,body pain,general health,energy,social function,emotional function and mental health of the two groups after treatment were higher than those before treatment（ P〈0. 05）. After treatment,the scores of physiological function,physiological function,body pain,general health,energy,social function,emotional function and mental health were all higher than those of the control group（ P〈0. 05）. Conclusion Zuogui-pill can effectively maintain the BMD of SOP patients,reduce their bone metabolic markers,increase serum β-catenin levels,and can also significantly improve the overall quality of life of patients
