2型糖尿病患者血清胆红素与颈动脉内膜中层厚度的关系

摘　　要：目的探讨2型糖尿病(T2DM)患者血清胆红素水平与颈动脉内膜中层厚度(CIMT)的关系。方法将72例T2DM患者依据CIMT值,分为内膜正常组(35例)、动脉硬化组(37例),记录患者的年龄、性别、糖尿病病程、吸烟史、体质量指数(BMI)、血压、空腹血糖(FBG)、糖化血红蛋白(Hb A1c)、血脂、血清胆红素水平,并进行统计学分析。结果动脉硬化组年龄(60.03±9.68)岁、糖尿病病程(7.81±5.63)年,显著高于内膜正常组年龄(52.29±7.46)岁、糖尿病病程(4.47±3.95)年,而动脉硬化组总胆红素(TBIL)、间接胆红素(IBIL)分别为(11.96±4.73)μmol·L-1、(8.86±3.61)μmol·L-1,低于内膜正常组的(14.05±3.98)μmol·L-1、(10.54±3.17)μmol·L-1,均差异有统计学意义(P<0.05)。血清胆红素水平与CIMT呈负相关(P<0.05)。结论 T2DM患者的胆红素水平变化与颈动脉内膜中层厚度密切相关。Objective To explore the relationship between serum bilirubin level and carotid intima-media thickness（CIMT） in type2 diabetes mellitus patients. Methods Seventy-two patients with T2 DM were assigned into the normal group（35 cases） and atherosclerotic group（37 cases） according to the value of CIMT. Patients’ age,gender,course of diabetes,smoking history,body mass index（BMI）,blood pressure,fasting plasma glucose（FBG）,Hb A1 c,lipid profiles and serum bilirubin were measured and analyzed.Results In the atherosclerotic group,patients’ age was（60. 03 ± 9. 68） years old and duration of diabetes was（7. 81 ± 5. 63）years,which were significantly higher than those in the normal group（52. 29 ± 7. 46） years old and diabetes duration of（4. 47 ±3. 95） years. Total bilirubin（TBIL） and indirect bilirubin（IBIL） were（11. 96 ± 4. 73） μmol·L-1 and（8. 86 ± 3. 61） μmol·L-1 in the atherosclerotic group,which were lower than those in the normal group,（14. 05 ± 3. 98） μmol·L-1 and（10. 54 ± 3. 17）μmol·L-1. There were significant differences（P 〈 0. 05）. Serum bilirubin level was negatively associated with carotid intima-media thickness（P 〈 0. 05）. Conclusions The change of serum bilirubin level in patients with T2 DM is closely related to the carotid intima-media thickness.
