PDCA循环管理在提高中药处方质量方面的应用价值

摘　　要：目的 :探讨PDCA循环管理在提高中药处方质量方面的应用价值。方法 :将2016年1月至2017年6月期间惠州市第一人民医院开具的5400张中药处方(每月随机抽选300张处方)作为研究对象。该院于2016年7月开始对中药处方进行PDCA循环管理。将2016年1月至6月期间(进行PDCA循环管理前)该院开具的1800张中药处方作为干预前组。将2016年7月至12月期间(进行第一轮PDCA循环管理后)该院开具的1800张中药处方作为第一轮循环组。将2017年1月至6月期间(进行第二轮PDCA循环管理后)该院开具的1800张中药处方作为第二轮循环组。比较三组处方的合理率及其中各项不合理情况的发生率。结果 :1)干预前组处方的合理率为92.33%,第一轮循环组处方的合理率为95.33%,第二轮循环组处方的合理率为97.94%。与干预前组处方相比,第一轮循环组处方和第二轮循环组处方的合理率均更高(P<0.01)。与第一轮循环组处方相比,第二轮循环组处方的合理率更高(P<0.01)。2)与干预前组处方相比,第一轮循环组处方和第二轮循环组处方中各项不合理情况的发生率均更低(P<0.05)。与第一轮循环组处方相比,第二轮循环组处方中各项不合理情况的发生率均更低(P<0.05)。结论 :PDCA循环管理在提高中药处方质量方面的应用价值较高,能够有效地提高中药应用的合理性。Objective To study Application value of PDCA cycle to continuously improv-e the quality of Chinese Herbal prescription. Method ： Select 5400 prescriptions made in The First Hospital of Huizhou City during January 2016 to June 2017（select 300 prescription every month）for research object. The hospital begin apply PDCA cycle in prescription management. Select 1800 prescriptions made during January to June in 2016（before application of PDCA cycle） for pre-intervention group. Select 1800 prescriptions made during June to December in 2016（After first PDCA cycle management）for 1 st cycle group. Select 1800 prescriptions made during January to June in 2017（After second PDCA cycle management） for 2 nd cycle group. Compare qualified rate and irrationality rate in 3 groups. Results 1）Qualified rate in pre-intervention group is 92.33%,in 1 st cycle group is 95.33%,in 2 nd cycle group is 97.94%.Compare with pre-intervention group, qualified rate in 1 st cycle group and 2 nd cycle group are higher（ P〈 0.01）.Compare with 1 st cycle group, qualified rate in 2 nd cycle group is higher（P〈 0.01）. 2）Compare with pre-intervention group, irrationality rate in in 1 st cycle group and 2 nd cycle group are lower（P 〈0.05）. Compare with 1 st cycle group, irrationality rate in 2 nd cycle group is lower（P 〈0.05）. Conclusion Application value of PDCA cycle to continuously improve the quality of Chinese Herbal prescription is high, it will improve rationality of prescription effectively.
