386例妇科急腹症病例特点分析

摘　　要：目的对妇科急腹症病例的病因、临床表现、诊断过程、治疗情况、手术以及患者结局等进行总结,为进一步提高妇科急腹症的诊治水平提供依据。方法回顾性分析某院2004年1月-2017年12月收治的386例妇科急腹症病例,包括疾病种类、构成比、临床表现、手术率、诊断符合率、治疗结果及预后等。结果 386例妇科急腹症病例,年龄13岁~62岁,平均(31±7.8)岁,385例治愈,1例好转。临床表现最常见的是急性腹痛、腹膜刺激征阳性、腹腔积液、急性失血表现,发生率均超过70%;最常见的病种是腹腔内出血性疾病295例,占全部病例的76.42%,肿瘤蒂扭转与破裂42例,占10.88%,感染性疾病33例,占8.55%。结论出血性妇科急腹症、蒂扭转、急性盆腔感染是常见妇科急腹症,妇科急腹症特殊情况下会发生误诊,在妇科急症处理中要加强多学科会诊,提高正确诊断率。Objectives To analyze the features of diagnosis and treatment of gynecological acute abdomen patients, improve the treatment effect. Methods To review and analyze the 386 gynaecological acute abdomen patients in the past 13 years in our hospital, including the type of disease, constituent ratio, clinical symptoms,operational ratio, accurate rate of diagnosis, therapeutic outcome and prognosis, etc. Results In the386 gynecological acute abdomen patients, aged from 13 to 62, average age was（31±7.8）, 385 got cured, and one got improved. Clinical symptoms of acute abdomen, positive for peritoneal irritation sign, pyoperitoneum, acutebleeding, the incidence occurrences are above 70%.Bleeding kind of diseases take up 76.42% of overall diseases. 42 tumor torsion and broken, 33 rational pelvic inflammation. Conclusions Bleeding kind of gynecological acute abdominal diseases were acute, develop quickly. The key to diagnosis and treatment was to clarify diagnosis and make a right treatment decision in time. Torsion was the most common acute abdominal disease. In case of complete torsion, emergency treatment was needed. Gynecology and surgery might be misdiagnosed at certain cases. In acute care of gynecological symptoms, multiple disciplinary consultation was necessary in order to improve the rate of correct diagnosis.
