摘　　要：目的了解陕西中医药大学附属医院职业性手外伤的流行病学特征及其直接经济负担,探讨职业性手外伤的预防控制策略。方法选择2013年1月—2015年12月在陕西中医药大学附属医院骨科诊断和治疗的职业性手外伤患者509例作为研究对象,记录患者一般情况、手损伤情况以及直接经济负担情况等,并进行描述分析。结果职业性手外伤患者男性多于女性,男女比例为3.31∶1,年龄集中在20~49岁(75.05%)。9月份职业性手外伤比例最高(12.18%),其次为3月份(10.81%);多发生在8:00~13:00(38.90%),特别是11:00~12:00(10.02%)。85.46%的患者可在职业性手外伤发生后8h内到我院就诊,就诊时间间隔平均为2.00(1.00,4.00)h。受伤手右手略高于左手,受伤部位以手指为主(77.93%),其中食指最多(19.67%),其次为中指(18.32%)和环指(15.92%);损伤组织前3顺位为皮肤(45.52%)、骨与关节(24.71%)和肌腱(15.86%)。患者直接经济负担平均为7208.54(5123.06,10946.71)元,其中治疗费比例较高。结论职业性手外伤有明显的性别、年龄和时间分布特点,食指受伤最多,受伤组织以皮肤损伤多见。[Objective]To know the epidemiological characteristics and its direct economic burden of occupational hand injury in affiliated hospital of Shaanxi university of Chinese medicine, and explore the prevention and control strategies of occupational hand injury. [Methods] A total of 509 cases of occupational hand injury patients diagnosed and treated in department of orthopedics in affiliated hospital of Shaanxi university of Chinese medicine from January 2013 to December 2015 were selected as research objects, the general conditions, hand injuries, and direct economic burden were recorded and analyzed using descriptive methods. [Results] The male patients of occupational hand injury were more than female, the ratio of male to female was 3.31 ∶1,the patients aged 20-49 years old was the most which accounted for 75.05%. Occupational hand injury happened in the September was the most（12.18%）, and the March followed（10.81%）. Occupational hand injury occurred highest in 8：00-13：00（38.90%）, especially for 11：00-12：00（10.02%）. 85.46% patients could visit doctor during eight hours after the occupational hand injury occurred, and the average interval of visits was 2.00（1.00, 4.00） hours. The injury right hand was slightly higher than the left hands. The fingers was mainly among injured parts（77.93%）, and the forefinger accounted mostly（19.67%）, followed by the middle finger（18.32%） and ring finger（15.92%）. The top three cis position of injury tissue was skin（45.52%）, bone and joint（24.71%） and tendon（15.86%）. The average of direct economic burden was 7208.54（5123.06, 10946.71） yuan, and the proportion of treatment costs was higher among them. [Conclusion] Occupational hand injury has obvious distribution of gender, age and time,the number of forefinger injury is the most, and the skin injury is common in injured tissue.
