摘　　要：目的探讨颈总动脉外膜剥脱术在混合型脑瘫外科治疗中的效果。方法对56例混合型脑瘫患儿行颈总动脉外膜剥脱术，术后根据流涎分级法（TDS）、改良Ashworth痉挛量表评定及综合脑功能障碍评定标准评价治疗效果。结果术后流涎改善情况：56例患儿，术后1个月显效7例，有效28例，无效21例；术后6个月显效9例，有效35例，无效12例。术后改良Ashworth痉挛量表评定情况：术后1个月显效4例，有效29例，无效23例；术后6个月显效6例，有效41例，无效9例。术后综合脑功能障碍改善情况：术后1个月显效7例，有效27例，无效22例；术后6个月显效10例，有效27例，无效19例。结论颈总动脉外膜剥脱术治疗混合型脑瘫效果明显，值得研究推广。Objective To investigate the effect of external carotid artery exfoliation on mixed cerebral palsy. Methods Fifty -six patients with mixed cerebral palsy were treated by external carotid artery exfoliation, and the therapeutic effect were classified as excellent, effective and ineffective, based on the assessment of salivary and modified Ashworth spasm scale and the improvement of integrated brain function. Results Postoperative improvement of salivary： among the 56 children, therapeutic effect was excellent in 7 cases, effective in 28 cases and ineffective in 21 cases 1 month after treatment; and it was excellent in 9 cases, effective in 35 cases and ineffective in 12 cases 6 months after treatment. Postoperative assessment by modified Ashworth spasm scale： treatment effect was excellent in 4 cases, effective in 29 cases and ineffective in 23 cases 1 month after surgery; and it was excellent in 6 cases, effective in 41 cases and ineffective in 9 cases 6 months after treatment. Postoperative improvement of integrated brain function： treatment effect was excellent in 7 cases, effective in 27 cases and ineffective in 22 cases 1 month after surgery; and it was excellent in 10 cases, effective in 27 cases and ineffective in 19 cases 6 months after treatment. Conclusions The effects of external carotid artery exfoliation on mixed cerebral palsy are remarkable and it is worthy of study
