凉血潜阳药物对寻常性银屑病血热证患者程序性死亡蛋白-1及其配体影响

摘　　要：目的探讨凉血潜阳药物对寻常性银屑病血热证患者皮损程序性死亡蛋白(PD)-1及其配体的影响。方法收集上海中医药大学附属岳阳中西医结合医院寻常性银屑病患者资料,分别选取血热证患者(血热证组)、血热证接受凉血潜阳药物治疗痊愈及显效患者(愈显组)、血热证接受凉血潜阳药物治疗无效患者(无效组)和非血热证患者(非血热证组)各6例,另选取健康对照受试者(健康对照组,6例)进行比较,采用实时聚合酶链反应(PCR)测定皮损处PD-1、程序性死亡蛋白配体-1(PD-L1)、程序性死亡蛋白配体-2(PD-L2)的mRNA表达量,蛋白质印迹法测定皮损处PD-1、PD-L1和PD-L2蛋白的表达。结果 PCR结果显示,与健康对照组比较,血热证组(下调为0.150)、无效组(下调为0.121)和非血热证组(下调为0.047)PD-1mRNA表达均降低,而愈显组PD-1mRNA表达上调为1.459,差异均具有统计学意义(P<0.01);与健康对照组比较,血热证组(下调为0.039)、无效组(下调为0.144)、非血热证组(下调为0.046)PD-L2mRNA表达均显著降低,愈显组PD-L2mRNA表达上调为1.075(P<0.01)。蛋白质印迹法检测PD-1、PD-L1和PD-L2蛋白表达结果显示,与健康对照组比较,愈显组PD-1蛋白表达升高,无效组和非血热证组PD-1蛋白表达下降;无效组和非血热证组PD-L1蛋白表达升高;血热证组和无效组PD-L2蛋白表达下降,愈显组PD-L2表达升高,差异均具有统计学意义(P<0.01)。结论凉血潜阳药物可能通过上调寻常性银屑病血热证患者皮损中PD-1mRNA及蛋白、PD-L2mRNA及蛋白的表达而发挥作用。Objective To define the effect of cooling blood and restrain Yang herbs on lesions of PD-1 and its ligands in the psoriasis vulgaris patients with blood-heat syndrome. Methods Hospital patients＇ data with psoriasis vulgaris in Shanghai university of traditional Chinese medicine affiliated Yueyang hospital of intrgrated traditional Chinese medicine and western medicine were selected and divided into 4 groups, such as the blood-heat syndrome group（blood-heat syndrome patients, n=6）; the cured group（blood-heat syndrome patients using cooling blood and restrain Yang herbs showed effective or completely cured, n=6）; the ineffective group（blood-heat syndrome patients receiving cooling blood and restrain Yang herbs showed no cured, n=6） and the non blood-heat syndrome group（non-blood heat syndrome patients, n=6）. The other selected healthy control subjects as control group（n=6）. ThemRNA and protein expression of PD-1, PD-L1 and PD-L2 in the lesions measured by Real time-PCR and Western Blot in each group were compared. Results PCR results showed that, compared with control group, the PD-1 mRNA expression in blood-heat syndrome group was down-regulated for 0.150, in ineffective group was down-regulated for 0.121, in non blood-heat syndrome group was down-regulated for 0.047, in cured group was up-regulated for 1.459, the differences were statistically significant（P〈0.01）.
