硫酸阿米卡星联合桂枝茯苓胶囊治疗慢性盆腔炎合并盆腔积液的疗效观察

摘　　要：目的探讨硫酸阿米卡星联合桂枝茯苓胶囊用于慢性盆腔炎合并盆腔积液治疗的临床效果。方法收集我院2014年8月至2017年2月期间住院接受治疗并诊断为慢性盆腔炎合并盆腔积液患者的病例资料,联合治疗组接受硫酸阿米卡星联合桂枝茯苓胶囊治疗,单药治疗组接受硫酸阿米卡星治疗,联合治疗组与单药治疗组各选取67例,比较两组治疗后疗效、疼痛评分、症状缓解时间、盆腔积液直径、住院天数、WHOQOL-BREF评分、炎性因子水平和不良反应以及复发率。结果联合治疗组治疗3 d和7 d后的痊愈率、显效率以及总有效率均高于单药治疗组,差异具有统计学意义(P<0.05);联合治疗组治疗后疼痛评分、症状缓解时间、盆腔积液直径和住院天数均低于单药治疗组,差异均具有统计学意义(P<0.05);联合治疗组WHOQOL-BREF各项评分中健康状况、生存质量、自我感觉和日常生活评分均高于单药治疗组,差异具有统计学意义(P<0.05);联合治疗组治疗后炎性因子C反应蛋白和肿瘤坏死因子-α均低于单药治疗组,白介素-2高于单药治疗组,差异具有统计学意义(P<0.05);联合治疗组治疗后不良反应发生率与单药治疗组基本持平,差异无统计学意义(P>0.05);且复发率低于单药治疗组,差异具有统计学意义(P<0.05)。结论硫酸阿米卡星联合桂枝茯苓胶囊治疗慢性盆腔炎合并盆腔积液,有助于缓解症状,缩短住院时间,减少炎性反应,且可提高患者生活质量,不良反应和复发率低,推荐临床广泛应用。Objective To observe the effect of amikacin sulfate combined with Guizhi Fuling capsule in the treatment of the chronic pelvic inflammation complicated with pelvic effusion. Methods A total of 134 patients of the chronic pelvic inflammation complicated with pelvic effusion in our hospital from August 2014 to February 2017 were selected and divided into combination therapy group and single drug group, 67 cases each group. The combination therapy group was treated with amikacin sulfate combined with Guizhi Fuling capsule, and the single drug group was treated with amikacin sulfate alone. The clinical outcomes, pain score, symptom relief time, pelvic effusion diamete, days of hospitalization, WHOQOLBREF score, inflammatory factor levels, adverse drug reactions, and recurrence rate were compared. Results After 3 days and 7 days of treatment, the cure rate, effective rate and total effective rate in combination therapy group were higher than those in single drug group, the difference was statistically significant（P〈0.05）. In the combination therapy group, the pain score, symptom relief time, pelvic effusion diamete and days of hospitalization were less than those in single drug group significantly（P〈0.05）. The health status, quality of life, self-perception and daily life score of the WHOQOL-BREF score in the combination therapy group were higher than those in single drug group significantly（P〈0.05）. The levels of CRP and TNF-α in the combination therapy group were lower than those in single drug group, and the IL-2 was higher than that in single drug group significantly（P〈0.05）. The incidence of adverse drug reactions in the combination therapy group was similar to that in single drug group, and the recurrence rate was lower than that of single drug group significantly（P〈0.05）. Conclusion Amikacin sulfate combined with Guizhi Fuling capsule is effective in the treatment of chronic pelvic inflammatory complicated with pelvic effusion, the symptoms are relieved, the days of hospitalizatio
