摘  要:目的分析抵抗素299GA、PPARγPro12Ala基因多态性与妊娠期糖尿病(GDM)患者肥胖的相关性。方法选取2015年2月-2017年2月该院收治的100例GDM孕妇作为GDM组,100例正常孕妇作为对照组。分别于两组研究对象空腹12 h后采集6 ml静脉血,分为两份,分别检测血糖相关指标[空腹血糖(FPG)、糖化血红蛋白(Hb A1c)、空腹胰岛素(FIns)、胰岛素抵抗指数(HOMA-IR)]及299GA、PPARγPro12Ala基因多态性。比较GDM组与对照组的299GA基因的GG、GA、AA基因型占比及G、A基因的频率分布与PPARγ基因的PP、PA、AA基因型占比及P、A基因的频率分布。比较GDM组及对照组中299GA基因与PPARγPro12Ala基因的不同基因型患者的血糖相关指标FPG、Hb A1c、FIns、HOMA-IR。分析GDM发病与GDM肥胖与299GA基因多态性及PPARγ基因多态性的相关性。结果 GDM组299GA基因的GG、GA、AA基因型占比分别为36.00%、52.00%、12.00%,对照组为42.00%、48.00%、10.00%(P>0.05)。GDM组299GA基因G、A基因频率分别为62.00%、38.00%,对照组为66.00%、34.00%(P>0.05)。GDM组中GG患者的BMI、FIns、HOMA-IR均低于GA基因型患者与AA基因型患者(均P<0.05),GA基因型患者体质量指数(BMI)、FIns、HOMA-IR低于AA基因型患者(P<0.05)。GDM组PPARγ基因的PP、PA基因型占比分别为86.00%、14.00%,对照组为92.00%、8.00%(P>0.05);P、A基因频率分别为93.00%、7.00%,对照组96.00%、4.00%(P>0.05)。GDM组中PP患者BMI高于PA基因型患者,FIns低于PA基因型患者(P<0.05)。GDM发病与299GA及PPARγ基因多态性之间均无相关性(P>0.05),GDM肥胖与299GA基因多态性呈明显正相关(r=0.397,P=0.030),与PPARγ基因多态性呈明显负相关(r=-0.432,P=0.021)。结论抵抗素299GA、PPARγPro12Ala基因多态性与GDM的发病无直接关系,但可通过影响患者IR而加重糖脂代谢紊乱,与GDM患者肥胖明显相关。Objective To analyze the correlations between gene polymorphisms of resistin 299 GA,PPARγPro12 Ala and obesity in gestational diabetes mellitus(GDM) patients. Methods A total of 100 pregnant women with GDM treated in Northwest Women ’ s and Children’s Hospital from February 2015 to February 2017 were selected as GDM group,and 100 normal pregnant women were selected as control group. The venous blood samples(6 ml) were collected after 12 hours of fasting in the two groups,blood glucose-related indexes[fasting plasma glucose(FPG),glycosylated hemoglobin(Hb A1 c),fasting insulin(FIns),HOMA-IR]and gene polymorphisms of resistin 299 GA,PPARγPro12 Ala were detected,respectively. The proportions of 299 GA GG,GA,and AA genotypes and distribution frequencies of G and A alleles,the proportions of PPARγ PP,PA,and AA genotypes and distribution frequencies of P and A alleles in GDM group and control group were compared. The levels of FPG,Hb A1 c,Fins,and HOMA-IR in pregnant women with different genotypes of299 GA and PPARγPro12 Ala in the two groups were compared. The correlations between onset of GDM and obesity,gene polymorphisms of299 GA and PPARγ were analyzed. Results The proportions of 299 GA GG, GA, and AA genotypes in GDM group were 36. 00%,52. 00%,and 12. 00%,respectively; the proportions of 299 GA GG,GA,and AA genotypes in control group were 42. 00%,48. 00%,and 10. 00%,respectively,there was no statistically significant difference between the two groups(P〉0. 05). The distribution frequencies of G and A alleles of 299 GA gene in GDM group were 62. 00% and 38. 00%,respectively; the distribution frequencies of G and A alleles of299 GA gene in control group were 66. 00% and 34. 00%,respectively,there was no statistically significant difference between the two groups(P〉0. 05). BMI,Fins,and HOMA-IR in pregnant women with GG genotype in GDM group were statistically significantly lower than those in pregnant women with GA genotype and pregnant women with AA genotype(P〈0. 05); BMI,Fins,and HOMA
