摘　　要：报道1例诊断为慢性颌下腺炎的Ig G4-RD(Ig G4-related disease,Ig G4-RD)并复习相关文献。患者71岁,男,双侧颌下区包块。手术行双侧颌下腺切除术,病理报告:慢性颌下腺炎。术后3月,查ANA抗体(+),抗SSA(+),白球比0.7,血清Ig G5 350mg/L,病变组织有淋巴细胞和细胞浸润结合临床确诊为Ig G4-RD。给予糖皮质激素联合免疫抑制剂治疗3月,症状部分消失,部分缓解,免疫学指标趋正常。结合文献复习,Ig G4-RD常出现多个器官受累或相继受累症状,及时行血清Ig G4及病理学检查可减少漏诊。本病对糖皮质激素治疗反应良好,及时规范治疗可控制病情发展甚至治愈。A case of IgG-related disease（IgG4-RD） is reported and the relevant literature is reviewed is this paper. A 71-year-old male patients underwent surgical treatment of bilateral submandibular gland resection for tumefaetion of the gland. Pathological examination showed chronic submandibular gland inflammation. 3 months after operation immunohistoehemical examination showed ana antibody （ ＋ ） , anti-SSA （ ＋ ）, baiqiubi 0.7, serum IgG 5, 350 mg/L and the infiltration of lymphocytes and cells in the pathological tissues. Clinical diagnosis of IgG 4-rd was then made. After treatment with glucoeorticoid combined with immunosuppressive therapy for 3 months no new pathological change was observed. Serum IgG and pathological examinations are recommended for the diagnosis of igG4-RD. Glucocorticoid treatment is effective for the treatment of the disease.
