摘  要:目的:研究锦红汤对脓毒症大鼠小肠Toll样受体4(toll-like receptor 4,TLR4)及其信号通路表达的影响。方法:将192只大鼠常规适应性喂养后按照每组32只随机分为正常组(Z)、模型组(M)、假手术组(J)、中药组(H)、西药组(T)、联合组(L)等6组。治疗组造模后即予相应药物干预1次,其余组均同时以等量生理盐水干预,每组大鼠再按0 h、2 h、6 h、24 h分为4个亚组,每亚组8只。在各个时间点采集标本,所有大鼠于标本采集完毕后处死。正常组:喂正常饲料。模型组:造模(CLP脓毒症模型),术后予生理盐水灌胃1次。假手术组:剖腹后只在盲肠根部穿入丝线,不行结扎和穿孔,术后予生理盐水灌胃1次。中药组:造模(CLP脓毒症模型),术后予锦红汤灌胃1次。西药组:造模(CLP脓毒症模型),术后予亚胺培南腹腔内注射1次。联合组:造模(CLP脓毒症模型),术后予锦红汤灌胃联合亚胺培南腹腔内注射1次。观察6组大鼠小肠组织形态学变化和TLR4及其信号通路相关基因蛋白表达的变化。结果:模型组2 h后肠绒毛出现排列混乱、缩短或变钝,6 h后上皮细胞水肿,部分脱落显著,黏膜下间质水肿,24 h后肠黏膜绒毛萎缩,而上皮细胞坏死,黏膜水肿和炎症细胞浸润。中药组、西药组和联合组24 h后肠黏膜表现与模型组比较有改善,但3组之间差异不大(P>0.05);大鼠小肠组织LPS受体(lipopolysaccharide,CD14)、Toll样受体4(TLR4)、肿瘤坏死因子α(tumor necrosis factorα,TNF-α)、核转录因子kappa B(nuclear factor kappa,NF-κB)表达的影响蛋白表达在24 h后正常组表达最低,模型组表达最高;与正常组、假手术组比较,模型组、西药组、中药组、联合组大鼠小肠组织CD14、TLR4、TNF-α、NF-κB蛋白表达均有不同程度的升高(P<0.05);西药组、中药组、联合组大鼠小肠组织CD14、TLR4、TNF-α、NF-κB蛋白表达较模型组呈一定程度的下降(P<0.05);西药组、中药组大鼠小肠Objective: To study influence of Jinhong Decoction(JHD) on small intestine Toll-like receptor 4(TLR4) and signal pathway expression of rats with sepsis. Methods: The experiment was divided into 6 groups: the normal group,the sham operation group,the model group,the western medicine group,TCM group and the combined group. The normal group was fed with ordinary feed,and the model group was made with CLP sepsis model. The treatment groups were given the corresponding drugs on the basis of the establishing model The morphological changes of the small intestine in the 6 groups,TLR4,and signal pathway related gene protein expression were observed by the 0 h,2 hours,6 hours and 24 hours respectively. Results: After 2 hours in the model group,intestinal villi appeared disordered,shortened or blunt. After 6 hours in the model group,the epithelial cells had edema,partially shedding,and submucous interstitial edema. After 24 hours,intestinal villi atrophy,epithelial cell necrosis,mucosal edema and infiltration of inflammatory cells appeared in the model group. The intestinal mucosa performance of TCM group,the western medicine decoction group and combined group after 24 hours improved compared with the model group,but there were no significant differences between the 3 groups. The expression of CD14,TLR4,TNF-a,NF-kappa B protein expression in the small intestine of rats was the lowest in the normal group after 24 hours,and the highest expression in the model group. Compared with the normal group and the sham operation group,the expression of CD14,TLR4,TNF-alpha and NF-kappa B protein in the small intestine tissues of the model group,the western medicine group,the TCM group and the combined group had different degrees of increase(P〈0. 05). The expression of CD14,TLR4,TNF-α、NF-κB protein in the small intestine of the western medicine group,the TCM group and the combined group showed a certain decrease compared with the model group,and the protein expression of CD14,TLR4,TNF-α、NF-κB in the small intesti
