不同促排卵方案对高黄体生成素(LH)多囊卵巢综合征(PCOS)不孕患者妊娠结局的对比分析

摘　　要：目的对比分析不同促排卵方案对高黄体生成素(LH)多囊卵巢综合征(PCOS)不孕患者妊娠结局的影响。方法收集2016年7月-2017年7月我院收治的200例促排卵指导同房高LH的PCOS不孕患者的临床资料,进行回顾性分析。依据用药方案的不同将这些患者分为注射用尿促性素(HMG)组(A组)、枸缘酸氯米芬片(CC)组(B组)、来曲唑片(LE)组(C组)、LE+HMG组(D组)和CC+HMG组(E组)五组,每组40例。对五组患者HCG日生殖激素水平、促排卵效果、OHSS发生率及妊娠结局进行统计分析。结果 HCG日D组、E组患者的LH、E2水平均显著高于A组、B组、C组,组间比较差异有统计学意义(P<0.05),T水平均显著低于A组、B组、C组,组间比较差异有统计学意义(P<0.05);D组患者的LH、E2水平又均显著高于E组,组间比较差异有统计学意义(P<0.05),T水平又显著低于E组,组间比较差异有统计学意义(P<0.05)。D组、E组患者的单卵泡排卵率均显著高于A组、B组、C组,组间比较差异有统计学意义(P<0.05),HCG日成熟卵泡数、内膜厚度均显著多于A组、B组、C组,组间比较差异有统计学意义(P<0.05);D组患者的单卵泡排卵率又显著高于E组,组间比较差异有统计学意义(P<0.05),HCG日成熟卵泡数、内膜厚度又均显著低于E组,组间比较差异有统计学意义(P<0.05),但五组患者的OHSS发生率组间比较差异无统计学意义(P>0.05)。D组、E组患者的平均周期数显著少于A组、B组、C组,组间比较差异有统计学意义(P<0.05),临床妊娠率均显著高于A组、B组、C组,组间比较差异有统计学意义(P<0.05);D组患者的平均周期数又显著少于E组,组间比较差异有统计学意义(P<0.05),临床妊娠率又显著高于E组,组间比较差异有统计学意义(P<0.05),但五组患者的流产率、多胎妊娠率组间比较差异无统计学意义(P>0.05)。结论 LE+HMG较CC+HMG、LE、CC、HMG促排卵方案更能有效提升高LH的PCOS不孕患者的LH、E2�Objective：To compare the effects of different ovulation induction protocols on pregnancy outcomes in patients with infertility caused by high luteinizing hormone（LH）and polycystic ovary syndrome（PCOS）. Methods：200 cases of patients with ovulation induction with LH-PCOS guide infertility in our hospital from July 2016 to July 2017 were randomly selected,according to the different regimens of these patients were divided into Menotropins for Injection（HMG）group（group A）,clomiphene citrate tablets（CC）group（group B）,Letrozol Tablets（LE）group（C group）,LE＋HMG group（D group）and CC＋HMG group（E group）five groups,40 cases in each group,the hormone levels,promoting ovulation,pregnancy outcome results of the five groups of patients were statistically analyzed. Results：The HCG,LH,E2 levels of D group,E group were significantly higher than A group,B group,C group,the differences between the groups were significant（P〈0.05）,the T levels were significantly lower than A group,B group,C group,the differences between the groups were significant（P〈0.05）;the LH and E2 levels of D group were significantly higher than E group,the differences between the groups were significant（P〈0.05）,the T level was significantly lower than E group,the difference between the groups was significant（P〈0.05）. The single follicle ovulation rates of D group,E group were significantly higher than A group,B group,C group,the differences between the groups were significant（P〈0.05）,the HCG number of mature follicles,endometrial thickness were significantly higher than A group,B group,C group,the differences between the groups were significant（P〈0.05）,the single follicle ovulation rate of D group was significantly higher than the E group,the difference between the groups was significant（P〈0.05）,the HCG number of mature follicles,endometrial thickness was significantly lower than E group,the difference between the groups was significant（P〈0.05）,but the differences of
