温阳益气汤联合西药治疗慢性心力衰竭的综合疗效观察

摘　　要：目的研究温阳益气汤联合西药治疗慢性心力衰竭(CHF)的临床疗效及对心功能的影响。方法选取2014年4月至2016年1月间收治的CHF患者114例,采用随机数字法将其分为对照组和观察组,每组57例。对照组患者接受利尿剂、强心剂、洋地黄类、螺内酯等西医常规治疗,在此基础上,观察组患者接受温阳益气汤治疗。观察两组患者的临床疗效和治疗前后心功能各指标[包括左室舒张末期内径(LVED)、左心室射血分数(LEVF)、心输出量(CO)及每分钟搏出量(SV)]情况,同时比较治疗前后各时间段生活质量评分和不良反应发生率。结果观察组的治疗总有效率(94.74%)明显高于对照组(73.69%),差异有统计学意义(P<0.05)。两组患者治疗后,观察组患者的LEVF、CO及SV水平均明显高于对照组(P<0.01);治疗后6、12及24周,观察组患者的生活质量评分均明显低于对照组(P<0.01)。两组患者治疗后均无明显不良反应发生。结论温阳益气汤联合西药治疗CHF的临床疗效显著,可有效改善患者的心脏功能,提高生活质量,且治疗不良反应较少,值得在临床推广。Objective To study the clinical therapeutic effects on chronic heart failure(CHF） treated with wenyang yiqi decoction and western medicines and the impacts on heart function. Methods From April 2014 to January2016,114 CHF patients were collected and divided into a control group and an observation group,57 cases in each one according to the random number table. In the control group,the routine western medicine was given,such as diuretic,cardiotonic,digitalis and spironolactone. On the base of the treatment as the control group,wenyang yiqi decoction was used in the observation group. The total effective rates and the heart function indexes before and after treatment(LVED,LEVF,CO and SV） were observed in the two groups. Before and after treatment,the score of living quality and the incidence of adverse reactions were compared. Results The total effective rate in the observation group(94. 74%） was higher apparently than the control group(73. 69%）,indicating the significant difference(P 〈 0. 05）. After treatment,the levels of LEVF,CO and SV in the observation group were higher obviously than the control group(P 〈 0. 01）. In 6,12 and 24 weeks after treatment,the score of m1 HFQ in the observation group was lower obviously than the control group(P 〈 0. 01）. There was no obvious adverse reaction after treatment in the two groups. Conclusion Wenyang yiqi decoction combined with western medicines achieves the significant clinical effects on CHF and effectively improves the heart functions and living quality in the patients. This therapy has less adverse reactions during the treatment and deserves to be promoted in clinical practice.
