腹部皮片在头面部皮肤癌术后软组织缺损修复中的应用

摘　　要：目的探讨腹部皮片修复头面部皮肤癌术后软组织缺损的可行性及疗效。方法选取在2013年7月至2016年7月本院口腔颌面外科应用腹部皮片修复面部皮肤癌术后软组织缺损的患者18例。所有患者均采用肿瘤扩大切除术,病灶切除后皮肤缺损范围为2cm×3cm～6cm×7cm。腹部皮片切取范围为2cm×3cm～7cm×8cm,腹部皮片移植同期修复术后面部软组织缺损,油纱加压缝合。结果 18例腹部皮片均顺利成活。所有患者均获随访,随访时间6～12个月,平均9个月,除1例死于鳞癌转移外,其余17例均无复发,皮片质地、色泽恢复良好。结论腹部皮片制备简便,修复头面部皮肤癌术后缺损成功率较高,是一种行之有效的方法。Objective To investigate the effect and feasibility of the reconstruction of tissue defect after excision of skin malignancies in the head and face. Method From July 2013 to July 2016, 18 patients with facial tissue defects were collected from our hospital. The soft tissue defects were repaired with abdominal razor-thin graft after skin malignancies excision. All cases were treated with extended resection of tumor, and the range of skin defect was from 2cm×3cm to 6cm × 7cm after resection. The abdominal skin grafts were designed and obtained from 2cm × 3cm to 7cm× 8cm, and the facial soft tissue defect was repaired with abdominal skin graft simultaneously. Result 18 cases skin grafts all survive with natural shapes and all patients were followed up for 6 to 12 months, with an average of 9 months. Only 1 case died of squamous cell carcinoma metastasis, the other 17 cases had no recurrence. Conclusion Abdominal skin graft preparation is simnle and convenient, and it is an effective method to renair the defect of head and face skin cancer after oneration.
