摘　　要：目的评价三维反转成像在产前诊断胎儿心房异构中的应用,总结其临床诊断价值。方法回顾性分析2011年6月-2017年7月间于深圳市龙岗中心医院和深圳市龙岗区妇幼保健院经二维超声联合三维超声诊断,经病理与出生后超声心动图确诊的80例异构综合征胎儿临床资料,观察对比二维超声与三维超声诊断各类心血管异常的符合率。结果二维超声诊断与三维超声反转成像的左侧异构综合征诊断符合率差异无统计学意义(P>0.05)。三维超声反转成像的右侧异构综合征诊断符合率及诊断完整性级别高于二维超声诊断,差异有统计学意义(P<0.05)。结论利用二维超声及三维超声反转成像显示心耳形态来确定心房反位或心房异构等先心病能提供直接证据,三维超声反转成像在诊断右侧异构房室异构等异常中表现更优。Objective To evaluate the application of three-dimensional（3 D） inversion imaging in prenatal diagnosis of fetal atrial isomerism, and to summarize its clinical diagnostic value. Methods To retrospectively analyze the clinical data of 80 cases of heteromerous fetuses diagnosed by two-dimensional（2 D） ultrasonography combined with 3 D ultrasound,pathology and postnatal echocardiography in Longgang Central Hospital and Shenzhen Maternal and Child Health Hospital from June 2011 to July 2017.The coincidence rate between 2 D ultrasonography and 3 D ultrasound in the diagnosis of various cardiovascular abnormalities were compared. Results There was no significant difference in the diagnostic coincidence rate of left isomeric syndrome between 2 D ultrasonography and 3 D ultrasound inversion imaging（P〈0.05）. The integrity level and diagnostic coincidence rate of right isomeric syndrome of 3 D ultrasound inversion imaging were higher than those of 2 D ultrasonography（P〈0.05）. Conclusion 2 D ultrasonography and 3 D ultrasound inversion imaging can provide direct evidence by displaying the auricle form to determine congenital heart disease such as situs inversus or atrial isomeric. 3 D ultrasound reversal imaging is better in the diagnosis of isomeric isomeric abnormalities in the right side.
