“451”模式在社区老年糖尿病患者健康管理中应用研究

摘　　要：目的探索"451"模式在社区老年糖尿病患者健康管理中的应用效果。方法选取巢湖市某社区2型糖尿病老年患者为研究对象,随机分为2组,对照组为常规健康管理,观察组采用"451"模式进行健康管理。结果干预后与对照组相比,观察组在血糖控制、糖尿病自我管理效能和糖尿病患者生活质量方面均优于对照组(P<0.05)。结论 "451"模式在社区老年糖尿病患者健康管理上有较高的应用价值。Objective To explore the application effect of ＂451＂model in the health management of elderly diabetic patients in the community.Methods A group of elderly patients with type 2 diabetes in Chaohu were selected and randomly divided into 2 groups. The control group was given routine health management,while the observation group adopted the ＂451＂mode for health management. Results After intervention,compared with the control group,the observation group was better than the control group in blood sugar control,diabetes self-management efficacy and quality of life of diabetic patients（ P〈0. 05）. Conclusions The ＂451＂model has a high application value in the health management of elderly diabetic patients in the community.
