摘 要:目的观察2型糖尿病视网膜病变(DR)患者血清胆红素、胆汁酸水平的变化,分析其与DR的相关性。方法选取2014年1月-2017年1月入住本院明确诊断为DR患者240例,根据眼底检查分成非增殖期2型糖尿病视网膜病变组(NPDR,n=132)与增殖期2型糖尿病视网膜病变组(PDR,n=108)组,同时选取同期住院无糖尿病视网膜病的2型糖尿病患者作为对照组(NDR,n=100);收集3组入组对象的年龄、性别、糖尿病病程、血压等一般资料,检测3组患者的生化指标、糖化血红蛋白,并对DR的危险因素进行多因素logistic回归分析。结果与NDR组比较,NPDR组、PDR组在糖尿病病程、收缩压(SBP)、空腹血糖(FPG)、餐后2小时血糖(2h PG)、糖化血红蛋白(HbA1C)、血尿酸(UA)、胱抑素C(Cys C)等方面均明显升高,差异有统计学意义(均P<0.05);NPDR组、PDR组血清总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)、总胆汁酸(TBA)均较NDR组显著降低,差异有统计学意义(均P<0.05);(3)logistic回归分析:FPG、HbA1C、Cys C、糖尿病病程均为DR的危险因素;而TBIL、DBIL、TBA均为DR的保护因素。结论 2型糖尿病视网膜病变血清TBIL、DBIL、TBA均明显降低,其是DR的保护的保护性因素。Objective To observe the changes of serum bilirubin and bile acid in patients with type 2 diabetic retinopathy,and analyze the correlation between serum bilirubin and bile acid and diabetic retinopathy in patients with type 2 diabetes.Methods There were 240 patients with type 2 diabetic retinopathy who were treated in the department of ophthalmology in the second hospital of Sanming from January 2014 to January 2017 were selected as study subjects. They were divided into non-proliferative diabetic retinopathy(NPDR,n = 132) group and the proliferative diabetic retinopathy(PDR,n = 108) group according to the result of fundus inspection. At the same time,100 patients with type 2 diabetes(non-diabetic retinopathy) were selected as the non-diabetic retinopathy group(NDR,n = 100). General data including age,sex,diabetes course,systolic blood pressure(SBP) and diastolic blood pressure(DBP) were collected. The automatic biochemical detector was used to measure the biochemical indicators(including liver function,blood fat and renal function),and the glycosylated hemoglobin was measured by high pressure liquid phase method. Contrast the different of biochemical indicators and glycosylated hemoglobin in the different group,and logistic regression analysis was used to analyze the risk factors of type 2 diabetic retinopathy. Results Compared with the non-diabetic retinopathy(NDR) group,the levels of diabetes course,Systolic blood pressure(SBP),Fasting plasma glucose(FPG),2 hours plasma glucose(2h PG),Glycosylated hemoglobin(HbA1c),blood uric acid and cystatin C in NPDR group and PDR group were significantly increased,the differences were statistically significant(all P〈0. 05). Compared with the NDR group,the levels of total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IBIL) and the total bile acid(TBA) in NPDR and PDR group were significantly lower than those in NDR group,and the differences were statistically significant(all P〈0.05).
