温州地区胆囊结石病相关危险因素的分析

摘　　要：目的研究胆囊结石的影响因素,为制定预防对策提供依据。方法回顾性分析2014年6月-2016年12月本院肝胆外科的333例患者的临床资料和问卷调查数据。结果实验组患者的BMI、SBP、DBP、FPG、TC、TG、LDL-C及DBIL、IBIL水平均高于对照组,HDL-C水平低于对照组,差异均有统计学意义(P<0.05)。经多因素Logistic回归分析结果表明,年龄、性别、BMI、SBP、DBP、FPG、TG、TC、LDL-C、DBIL、IBIL、锻炼、荤素、家族史均是胆囊结石的影响因素(P<0.05),其中LDL、喜食荤食、有家族史、TG、DBIL、FPG、BMI、SBP、年龄为胆囊结石的独立危险因素,而TC、男性、锻炼、IBIL、DBP为保护性因素。结论胆囊结石的发生是多种因素共同作用的结果,在日常生活中,可通过监控血脂、血糖水平,调节饮食,控制体质量,必要时接受降糖降脂药物治疗等来预防胆囊结石的发生。Objective To research the influence factors of gallstone,and provide basis for the formulation of preventive measures. Methods Clinical data and questionnaire data of 333 patients in the Department of Hepatobiliary Surgery of our hospital during June 2014-December 2016 were retrospectively analyzed. Results The levels of BMI,SBP,DBP,FPG,TC,TG,LDL-C and DBIL and IBIL in the experimental group were all higher than those in the control group,and the level of HDL-C was lower than that in the control group,with the differences statistically significant（ P〈0. 05）. Multivariate Logistic regression analysis showed that age,gender,BMI,SBP,DBP,FPG,TG,TC,LDL-C,DBIL,IBIL,exercise,meat and family history were all influencing factors of gallstones（ P〈0. 05）. LDL,eating meat,family history,TG,DBIL,FPG BMI,SBP and age were independent risk factors of gallbladder stones,while TC,IBIL,DBP,male,exercise were protective factors. Conclusion Gallstone is the result of multiple factors. In daily life,we can prevent gallstone by monitoring blood lipid and blood sugar level,adjusting diet,controlling body weight,and taking hypoglycemic and lipid-lowering drugs if necessary.
