葛根芩连汤方证要义与合方应用

摘　　要：葛根芩连汤为临床常用经方,其组方精当,配伍严明,疗效显著。葛根芩连汤出自《伤寒论》第34条,历代医家对于本方主治是否有表证,众说纷纭、莫衷一是。通过文献考证、梳理辨析得出:本方虽为表里双解之剂,但是其清里热之功尤著,用于热利而兼表证者,可收解表清热之功;用于热利而无表证者,亦能获清热止利之效,因而应用本方当以身热下利为要点,不必拘泥于有无表证。同时,对葛根芩连汤的组方特点、药对药组,以及合方化裁进行了详实解读,并结合临床治验,以葛根芩连汤合方化裁治疗慢性肠炎、痔疮、溃疡性结肠炎、糖尿病等4个典型案例为佐证,介绍了葛根芩连汤的临床应用要点,以期对提高临床疗效有所裨益。Gegen Qinlian decoction, a classic decoction in traditional Chinese medicine, is wildly used clinically for its reasonable composition and precise curative effect. Based on article 34 of the Treatise on Cold Damage, it is hard to say whether Gegen Qinlian decoction was able to treat exterior syndrome. As opinions vary, no unanimous conclusion can be drawn. Although this decoction is famous for its effect to treat relieving exterior-interior syndrome, people believe it is preferable to treat internal heat syndrome after analyzing classic literature and theoretical evidence, especially heat diarrhea combined with exterior syndrome. Actually, fine curative effect could also be observed treat heat diarrhea without exterior syndrome. Therefore, the core symptoms of Gegen Qinlian decoction is heat diarrhea, so called large intestine damp-heat, no matter whether exterior syndrome exists. In the meanwhile, this article refers to application of Gegen Qinlian decoction and its combined decoction in four typical diseases, including chronic enteritis, haemorrhoids, ulcerative colitis and diabetes, and aims to evaluate the clinical application and benefit of Gegen Qinlian decoction.
