摘  要:目的探讨低辐射剂量CT扫描技术生成原始DICOM数据行冠状动脉3D建模打印的可行性研究。方法将2016年6月-2018年1月在南京总医院拟行冠脉3D建模打印患者,随机分为两组,每组10例。组1(低剂量组):低管电压:100kVp;低对比剂浓度:碘克沙醇(Iodixanol 270 mgI/mL);采用自适应统计迭代算法;组2(常规剂量组):常规管电压:120kVp;高对比剂浓度:碘佛醇(Ioversol 350mgl/mL);采用FBP重建算法。扫描结束后,分别计算两组的有效辐射剂量(effective dose,ED)值及体型特异性剂量评估(size—specific dose estimates,SSDE)值。将两组原始数据传至3D打印机行冠脉3D建模打印,并由两名资深3D打印工程师完成对两组方案获取的冠脉3D建模打印质量的主观评价。结果两组间患者所受ED、SSDE比较,低辐射剂量组较常规辐射剂量组显著降低(39%、37%),差异均有统计学意义(P〈0.05)。两组冠脉3D建模打印质量主观评分比较差异均无统计学意义(P〉0.05)。结论对于BMI指数在20~25kg/cm2之间的患者,应用低剂量CT扫描技术生成原始DICOM数据行冠脉3D建模打印能够满足临床医生需求,同时可大幅度降低患者所受的CT辐射剂量。Objective To explore the feasibility of coronary artery 3D modeling print based on low radiation dose CT scanning to generate original DICOM data. Methods 20 patients with coronary artery 3D printing were randomly divided into two groups, 10 cases in each group. Group 1 (low dose group): low tube vohage (100 kVp), low concentration contrast (iodixanol 270 mgI/ml) and adaptive statistical iterative reconstruction (ASiR). Group 2 (conventional dose group): conventional tube voltage (120 kVp), high concentration of contrast agent (ioversol, 350mgI/ ml) and filtering back projection (FBP). Two groups of raw data were sent to the 3D printer to perform coronary artery 3D modeling print, and the subjective scoring of the print model was completed. Results The ED and SSDE of the two groups were significantly lower than those of the conventional dose strategy group (39%, 37%), and the differences were statistically significant (P〈 0.05). There was no significant difference between the two groups in subjective evaluation of 3D modeling print quality (P 〉 0.05). Conclusion To those whose BMI between 20~25 kg/m2, applied the low dose CT scanning technique to generate original DIOCM data for coronary artery 3D modeling print can meet the needs of clinical diagnosis, and can greatly reduce the radiation dose to patients.
