PFNA与DHS内固定在股骨粗隆间骨折患者中的疗效对比观察

摘　　要：目的对比观察采用动力髋螺钉(DHS)与股骨近端防旋髓内钉(PFNA)内固定的方法治疗股骨粗隆间骨折的临床效果。方法收治96例股骨粗隆间骨折老年患者,47例实施PFNA,49例实施DHS内固定,对比两组手术效果、术后髋关节功能恢复情况及并发症发生率。结果 PFNA组手术时间、下床负重时间和DHS组比较明显较短,术中出血量和DHS组比较明显较少,差异有统计学意义(P<0.05)。PFNA组术后3个月、6个月Harris评分和DHS组比较明显较高,差异有统计学意义(P<0.05)。结论采用PFNA内固定治疗股骨粗隆间骨折具有良好的临床效果,与DHS比较,PFNA内固定治疗的手术时间短、并发症少,术后恢复快,特别适用于不稳定型股骨粗隆间骨折合并骨质疏松的老年患者。Objective To observe the clinical effect of dynamic hip screw（DHS） and proximal femoral intramedullary nail（PFNA）fixation in the treatment of intertrochanteric fractures.Methods Forty-nine elderly patients with intertrochanteric fractures were treated with PFNA.Forty-seven patients underwent DHS internal fixation.The operative outcomes,the recovery of hip function and the incidence of complications were compared between the two groups.Results There was a significant difference between the operation time and the DHS group in the PFNA group,and the difference between the two groups was statistically significant（P〈0. 05）.The Harris score and DHS group were significantly higher at 3 months and 6 months after PFNA group（P〈0. 05）.Conclusion PFNA internal fixation has a good clinical effect in the treatment of intertrochanteric fractures.Compared with DHS,PFNA internal fixation has short operative time,less complication and quick recovery.It is especially suitable for unstable femoral intertrochanteric Fractures associated with osteoporosis in elderly patients.
