摘 要:目的评价右美托咪定对术前睡眠剥夺大鼠术后痛的影响。方法健康成年雄性SD大鼠50只,12-14周龄,体重200-300 g,采用随机数字表法将其分为5组(n=10):对照组(C组)、睡眠剥夺组(SD组)、切口痛组(IP组)、睡眠剥夺+切口痛组(SD+IP组)和睡眠剥夺+切口痛+右美托咪定组(SD+IP+DEX组)。采用小平台水环境法建立大鼠睡眠剥夺模型,于第1天睡眠剥夺完成后制备切口痛模型。SD+IP+DEX组切口痛造模前腹腔注射右美托咪定50 μg/kg,连续3 d,其余组腹腔注射等容量生理盐水。于术前或睡眠剥夺前1 d、术后或睡眠剥夺后12、24和72 h时测定机械痛阈和热痛阈;行为学测试完毕后,采集血样和脊髓背角组织,采用ELISA法测定血清皮质酮浓度,采用高效液相色谱法测定脊髓背角5-羟色胺(5-HT)含量。结果与C组比较,其余4组机械痛阈和热痛阈降低,血清皮质酮浓度及脊髓背角5-HT含量升高(P〈0.05);与IP组比较,SD+IP组机械痛阈和热痛阈降低,血清皮质酮及脊髓背角5-HT含量升高,SD+IP+DEX组机械痛阈和热痛阈升高(P〈0.05),血清皮质酮及脊髓背角5-HT含量差异无统计学意义(P〉0.05);与SD+IP组比较,SD+IP+DEX组机械痛阈和热痛阈升高,血清皮质酮及脊髓背角5-HT含量降低(P〈0.05)。结论右美托咪定可减轻术前睡眠剥夺大鼠术后痛,机制可能与抑制应激反应和脊髓背角5-HT水平有关。Objective To evaluate the effect of dexmedetomidine on postoperative pain in rats with preoperative sleep deprivation.Methods Fifty healthy adult male Sprague-Dawley rats, aged 12-14 weeks, weighing 200-300 g, were divided into 5 groups (n=10 each) using a random number table: control group (group C), sleep deprivation group (group SD), incisional pain group (group IP), sleep deprivation plus incisional pain group (group SD+ IP) and sleep deprivation plus incisional pain plus dexmedetomidine group (group SD+ IP+ DEX). Sleep deprivation was induced by the flower pot technique, and then the incisional pain model was carried out on first day after completion of sleep deprivation.Dexmedetomidine 50 μg/kg was intraperitoneally injected for 3 consecutive days before establishing the model of incisional pain in group SD+ IP+ DEX, and the equal volume of normal saline was given in the other groups.The mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal threshold (TWT) were measured before operation or at 1 day before sleep deprivation and after operation or at 12, 24 and 72 h after sleep deprivation.Blood samples were collected and spinal cord tissues were removed after the end of behavior test for determination of serum corticosterone concentrations (by enzyme-linked immunosorbent assay and content of 5-hydroxytryptamine (5-HT) in spinal dorsal horns (by high-performance liquid chromatography).Results Compared with group C, the MWT and TWT were significantly decreased, and the serum corticosterone concentrations and content of 5-HT in spinal dorsal horns were increased in the other 4 groups (P〈0.05). Compared with group IP, the MWT and TWT were significantly decreased, and the serum corticosterone concentrations and content of 5-HT in spinal dorsal horns were increased in group SD+ IP, and the MWT and TWT were significantly increased (P〈0.05), and no significant change was found in the serum corticosterone concentrations or content of
