不同序贯疗法联合“益肾通淋汤”治疗输尿管结石临床研究

摘　　要：目的观察"益肾通淋汤"联合体外冲击波碎石术(Extracorporeal shock wave lithotripsy,ESWL)和输尿管镜气压弹道碎石术(Ureteroscopic Lithotripsy,URL)序贯疗法治疗输尿管结石的临床疗效。方法 194例输尿管结石患者分为结石直径≤10 mm和>10 mm组。直径≤10 mm先给予ESWL治疗,术后患者被分为常规治疗与加用中药方两组,中药方组配合中药治疗2周。结石未排出者以及结石直径>10 mm者行URL术,术后被分为常规治疗组与加用中药方两组。分别评估四组患者的临床疗效。结果 144例患者接受ESWL治疗,治疗组总有效率为77.8%(56/72),明显大于对照组的61.1%(44/72),两组比较差异有统计学意义(P<0.05)。150例患者接受输尿管镜碎石治疗,治疗组患者在术后2周、4周、6周残石率明显少于对照组(P<0.05)。结论 "益肾通淋汤"联合ESWL术和URSL术序贯疗法治疗输尿管结石体现了中西医结合治疗的优势。Objective To observe the clinical effects of sequential therapy of Herbs of Yi-kidney Tonglin,combined with extracorporeal shock wave lithotripsy and Ureteroscopic Lithotripsy on ureteral calculi. Methods A total of 194 patients with ureteral calculi were included,and they were divided into stone diameter ≤10 mm and 〉10 mm parts. Stone diameter ≤10 patients were treated with ESWL,and after operation they were divided into routine treatment and traditional Chinese medicine addition group. Patients with unexpelled stones or with stone diameter of more than 10 mm were treated with URSL. The patients were divided into regular treatment group and regular treatment combined with Herbs of Yi-kidney Tonglin group after lithotripsy. The clinical effects of four groups were assessed. Results 144 patients received ESWL treatment. The total effective rate of treatment group was 77. 8%（ 56/72）,which was significantly higher than the control group 61. 1%（ 44/72）. The difference between the two groups was statistically significant（ P〈0. 05）. 150 patients received URSL. The rate of residual stone 2,4,6 weeks after surgery of treatment group were significantly less than the control group（ P〈0. 05）. Conclusion Herbs of Yi-kidney Tonglin combined with ESWL and URSL sequential therapy in the treatment of ureteral calculi reflects the advantages of integrated traditional Chinese and Western medicine treatment.
