急性脑卒中患者疾病不确定感与焦虑、抑郁的相关性

摘　　要：目的：探讨急性脑卒中患者疾病不确定感与焦虑、抑郁的状况及其关系。方法：采用中文版疾病不确定感量表（MUIS-A）、状态焦虑问卷（S-AI）、自评抑郁量表（SDS）对住院的414名急性脑卒中患者进行问卷调查,分析患者疾病不确定感与焦虑、抑郁的相关性。结果：95.2%的脑卒中患者疾病不确定感处于中等水平,47.6%的患者存在高状态焦虑,58.7%的患者存在抑郁症状,疾病不确定感总分及其两个维度与焦虑、抑郁总分均呈正相关（r=0.189-0.584,P〈0.01）,低水平疾病不确定感与抑郁总分呈正相关（r=0.725,P〈0.05）,中等水平疾病不确定感与焦虑、抑郁总分呈正相关（r=0.138-0.173,P〈0.01）。结论：急性脑卒中患者普遍存在疾病不确定感,焦虑及抑郁状态检出率高,医护人员应该正确评估患者情况,及时做好知识宣教及心理疏导,降低患者的疾病不确定感及焦虑、抑郁情绪。Objective：To explore the status of illness uncertainty and the problem of anxiety and depression emotions and their correlation among hospitalized patients with acute stroke.Methods：Mishel Uncertainty in Illness Scale for Adults,State Anxiety Inventory and Self-Rated Depression Scale were used to assess 414 hospitalized patients with acute stroke.Results：A total of 95.2% and 2.9% patients with acute stroke had illness uncertainty at medium and high level,respectively.Nearly a half of them had anxiety disorder and 58.7% of them had depressive symptoms.The uncertainty in illness and its two dimensions were significantly positively correlated with anxiety and depression scores（r=0.189-0.584,P〈0.01）.The low level of uncertainty was positively correlated with depression score（r=0.725,P〈0.05）,and the middle level of uncertainty was positively correlated with anxiety and depression scores（r=0.138-0.173,P〈0.01）.Conclusion：The incidence of illness uncertainty,anxiety,and depression is high among hospitalized patients with acute stroke.It is necessary to strengthen health education and psychological support to decrease the illness uncertainty and the problem of anxiety and depression emotions among patients with acute stroke.
