糖尿病患者发生泌尿系统感染影响因素分析及血清PCT与CRP和血WBC水平的变化研究

摘　　要：目的分析糖尿病患者发生泌尿系统感染的影响因素及血清PCT、CRP和血WBC水平的变化情况。方法选取2012年12月-2017年1月医院收治的糖尿病患者1 330例为研究对象,统计糖尿病患者的临床感染情况,采用单因素分析和多因素回归统计分析导致糖尿病患者并发泌尿系统感染的影响因素,观察比较感染患者和非感染患者的血清PCT、CRP及血WBC等指标。结果 1 330例糖尿病患者发生医院感染102例,感染率为7.67%,其中泌尿系统感染39例占38.24%;多因素Logistic回归分析结果显示,年龄≥40岁、住院时间≥10d、病程≥6年、空腹血糖≥10mmol/L、Ca降低、糖化血红蛋白升高、低蛋白血症是糖尿病患者发生泌尿系统感染的影响因素(P<0.05);感染患者的PCT、CRP及血WBC指标均高于未感染患者(P<0.05)。结论糖尿病发生泌尿系统感染的影响因素较多,应引起临床重视,患者感染后血清PCT、CRP及血WBC水平会有所升高。OBJECTIVE To investigate the influencing factors for urinary tract infections in diabetes mellitus patients and observe the changes of levels of serum PCT,CRP and WBC.METHODS A total of 1 330 diabetes mellitus patients who were treated in the hospital from Dec 2012 to Jan 2017 were recruited as the study objects,the incidence of clinical infection was statistically analyzed,univariate analysis and multivariate regression analysis were performed for the influencing factors for urinary tract infections in the diabetes mellitus patients,and the levels of serum PCT,CRP and WBC were observed and compared between the patients with infections and the patients without infection.RESULTS Of the 1 330 diabetes mellitus patients,102 had nosocomial infections,with the infection rate 7.67%,39（38.24%）of whom had urinary tract infections.The result of multivariate logistic regression analysis showed that the influencing factors for the urinary tract infections in the diabetes mellitus patients included no less than 40 years of age,length of hospital stay no less than 10 days,course of disease no less than 6 years,fasting blood glucose no less than 10 mmol/L,reduction of Ca,elevated glycosylated hemoglobin and hypoproteinemia（P〈0.05）.The levels of PCT,CRP and WBC of the patients with infections were significantly higher than those of the patients without infection（P〈0.05）.CONCLUSIONThere are a variety of influencing factors for the urinary tract infections in the diabetes mellitus patients,it is necessary for the hospital to pay great attention to the influencing factors.The levels of serum PCT,CRP and WBC of the patients with infections are reduced.
