祛腐生肌膏联合五水头孢唑林钠对髋关节置换术后切口感染临床疗效观察

摘　　要：目的探讨祛腐生肌膏联合五水头孢唑林钠对髋关节置换术后切口感染的临床疗效。方法选取2013年12月-2017年1月医院收治的髋关节置换术后切口感染患者152例,按照随机数字表法分为对照组和试验组,各76例,对照组患者予以五水头孢唑林钠治疗,试验组在对照组治疗基础上予以祛腐生肌膏外敷,观察并记录两组间血清细胞因子和感染相关指标水平,比较两组患者临床疗效及不良反应发生情况。结果两组患者治疗前血清细胞因子及感染相关指标水平比较差异无统计学意义,治疗后所有指标水平均降低(P<0.05),且试验组指标水平优于对照组(P<0.05);试验组临床疗效优于对照组(Z=6.583,P=0.034);两组患者均出现头晕、嗜酸性粒细胞增高和创口出血等不良反应,症状均较轻微,但试验组有4例患者出现切口局部皮肤过敏瘙痒症状。结论祛腐生肌膏联合五水头孢唑林钠对髋关节置换术后切口感染的临床疗效较好,能有效促进切口愈合,降低炎症细胞因子水平,减轻炎症反应程度,抑制感染,而且安全性较高。OBJECTIVE To explore the clinical effect of Qufushengji ointment combined with cefazolin sodium pentahydrate on treatment of postoperative incision infections in patients undergoing hip arthroplasty.METHODS From Dec 2013 to Jan 2017,a total of 152 patients who underwent the hip arthroplasty and had postoperative incision infections were enrolled in the study and randomly divided into the control group and the experimental group,with 76 cases in each group.The control group was treated with cefazolin sodium pentahydrate,while the experimental group was given external application of Qufushengji ointment on basis of the treatment of the control group.The levels of serum cytokines and infection-related indexes of the two groups of patients were recorded and observed,and the clinical therapeutic effect and incidence of adverse reactions were compared between the two groups of patients.RESULTS There were no significant differences in the levels of serum cytokines and infection-related indexes between the two groups of patients before the treatment,the levels of all the indexes were reduced after the treatment（P〈0.05）,the levels of the indexes of the experimental group were significantly better than those of the control group（P〈0.05）.The clinical therapeutic effect of the experimental group was significantly better than that of the control group（Z=6.583,P=0.034）.The two groups of patients had the adverse reactions such as dizziness,increased eosinophils and wound bleeding,with the symptoms mild,however,4 patients had local skin allergies and itching at incision site in the experimental group.CONCLUSIONQufushengji ointment combined with cefazolin sodium pentahydrate has remarkable clinical effect on treatment of the hip arthroplasty patients with postoperative incision infections,effectively promotes the incision healing,reduces the levels of inflammatory cytokines,alleviates the inflammatory reactions and inhibits the infections,with the safety high.
