摘 要:目的了解某院主要革兰阴性菌药物敏感率的变迁,分析敏感率的变化与抗菌药物使用强度的相关性,为抗菌药物的合理应用和细菌耐药率的控制提供依据。方法采用回顾性调查方法,收集整理2013年1月~2017年12月该院主要革兰阴性菌对常见抗菌药物的敏感率和抗菌药物的使用强度,采用Pearson相关性分析法分析药物敏感率与使用强度之间的相关性。结果大肠埃希菌和肺炎克雷伯菌对头孢吡肟的敏感率与使用强度呈显著正相关(r=0.795,P=0.006;r=0.716,P=0.020);大肠埃希菌对头孢曲松钠的敏感率与使用强度呈显著正相关(r=0.675,P=0.032);铜绿假单胞菌对哌拉西林钠/他唑巴坦钠和庆大霉素的敏感率与使用强度呈显著负相关(r=-0.694,P=0.026;r=-0.682,P=0.030)。结论该院主要革兰阴性菌敏感率与部分抗菌药物的使用强度存在显著的相关性,合理使用抗菌药物可有效提高药物作用的敏感程度。OBJECTIVE In order to provide reference for rational use of antibiotics and control of bacterial resistance,the change trends of drug susceptibility rate about gram-negative bacterias and the correlation between bacterial susceptibility and antibiotics use density( AUD) were analyzed. METHODS Retrospective analysis was used for this study. Main antimicrobial susceptibility of gram-negative bacterias and AUD during January 2013 to December 2017 were collected. The correlation between drug sensitivity and use density was analyzed by Pearson correlation analysis. RESULTS The susceptibility rate of Escherichia coli and Klebsiella pneumoniae to cefepime had significant positive correlation with AUD of cefepime,the correlation coefficient of r value and P value were( 0.795,0.006) and( 0.716,0.020) respectively. Susceptibility rate of Escherichia coli to ceftriaxone sodium had significant positive correlation with AUD. The susceptibility rate of Pseudomonas aeruginosa to piperacillin sodium/tazobactam sodium and gentamicin had significant negative correlation with their AUD,r value and P value were(-0. 694,0. 026) and(-0. 682,0. 030) respectively.CONCLUSION The sensitivity rate of gram-negative bacterias in a hospital are significant correlated with AUD of some antibiotics.Rational use of antibiotics can effectively improve the drug susceptibility rate.
