体脂含量与胃癌发生的孟德尔随机化研究

摘　　要：[目的]采用孟德尔随机化方法,分析体脂含量与胃癌发生之间是否存在潜在的因果关联。[方法]采用病例对照研究设计,利用2 631例胃癌病例和4 373例对照的基因型数据,以全基因组关联研究已报道的10个体脂含量相关遗传变异作为工具变量,构建遗传评分,进一步采用多因素logistic回归模型评估体脂含量相关遗传变异及其遗传评分与胃癌发生风险的关联。[结果]多因素logistic回归模型分析发现,体脂含量相关遗传变异位点rs9906944(OR=1.265,95%CI:1.075~1.494,P=0.005)、rs543874(OR=1.108,95%CI:1.018~1.206,P=0.017)与胃癌发生风险相关。体脂含量遗传评分高的人群发生胃癌的风险较高,每增加一个升高体脂含量的等位基因,胃癌的发生风险增加4.1%(0.9%~7.4%,P=0.012)。[结论]体脂含量相关遗传变异及其遗传评分与胃癌发生风险相关,提示高体脂含量与胃癌发生之间可能存在潜在的因果关联。[Objective] In the current study, we used Mendelian randomization analysis to investigate whether body fat percentage （BF%） is causally associated with gastric cancer risk. [Methods] We used genetic data from 2 631 patients with gastric cancer and 4 373 controls, and constructed a genetic risk score （GRS） using I0 BF%-associated single nucleotide polymorphisms（SNPs）. We used multivariable logistic regression models to estimate associations of BF%- associated SNPs and GRS predicting BF% with risk of gastric cancer. [Results] BF%-associated SNPs rs9906944 and rs543874 were associated with gastric cancer risk（for rs99069d4, OR=1.265, 95% CI： 1.075~1.494, P=0.005; for rs543874, OR=l.108, 95% CI： 1.018~1.206, P=0.017）. High genetically predicted BF% was associated with increased gastric cancer risk. With each BF%-increasing allele, the occurrence risk of developing gastric cancer increased 4.1% （0.9%-7.4%, P=-0.012）. [Conclusion] Our analysis indicated that genetically predicted body fat percentage was associated with gastric cancer risk, suggesting that high body fat maybe causally associated with gastric cancer risk.
