摘　　要：目的探讨带尾孔针钛缆张力带治疗儿童尺骨鹰嘴骨折的临床疗效。方法自2013年6月至2016年12月，我们采用带尾孔针钛缆张力带内固定治疗尺骨鹰嘴骨折患儿15例，其中合并桡骨颈骨折2例，尺骨冠状突骨折1例，肘关节脱位1例。术后行石膏托外固定，4周后拆除，行肘关节主动功能训练。结果术后切口均Ⅰ期愈合。X线片显示骨折均复位良好，关节面平整，骨折均愈合，愈合时间为7~10周，平均8．4周。所有患儿均获得随访，时间为6~19个月，平均11个月。肘关节屈伸活动度平均为121．8°，旋转活动度平均为145．1°。采用Mayo肘关节功能评分（Mayo elbow performance score，MEPS）进行功能评估，MEPS评分为66~99分：优11例，良3例，可1例。术后无一例发生内植物松动、移位或断裂及退针。未发生骨不连、骨折再移位及医源性神经损伤等并发症。结论采用带尾孔针钛缆张力带治疗儿童尺骨鹰嘴骨折，操作简单，固定可靠，术后肘关节功能恢复良好，并发症少。Objective To evaluate the clinical effects of tailed needle titanium cable tension band for fixation of olecranon fractures in children. Methods From June 2013 to December 2016, 15 children with olecranon fractures were treated by internal fixation using tailed needle titanium cable tension band. There were 2 cases of radial neck fractures, 1 case of ulnar coronoid fracture and 1 case of elbow joint dislocation. Postoperatively external fixation was performed using plaster for 4 weeks, and elbow joint active function training was conducted after plaster removal. Results All incisions achieved primary healing. The postoperative X ray showed that all the fractures healed and the articular surface was smooth. The healing time ranged from 7 to 10 weeks with an average of 8.4 weeks. All the children were follow-up for 6 to 19 months with an average of 11 months. The average flexion and extension of elbow joint was 121.8°, and the average rotation activity was 145.1°. The function evaluation was performed by Mayo elbow performance score （MEPS）. The MEPS ranged fi：om 66 to 99. The results were excellent in 11 cases, good in 3 cases and fair in 1 case. No implant lossening, dislocation, fracture and needle withdrawal occurred postoperatively. No complications such as bone nonunion, fracture redisplacement and iatrogenic nerve injury occurred. Conclusion The treatment of olecranon fractures in children with tailed needle titanium cable tension band is simple and reliable. The function of elbow joint can achieve good recovery afier the operation with less complication.
