火针治疗慢性湿疹临床疗效及患者EASI、瘙痒评分、血清因子水平影响

摘　　要：目的：观察用火针治疗慢性湿疹的临床疗效及对患者湿疹面积和严重程度指数（EASI）、瘙痒评分、血清因子水平的影响。方法：66例慢性湿疹患者随机分为对照组和治疗组各33例。对照组患者给予盐酸西替利嗪片口服;治疗组患者则加用火针治疗,连续治疗1个月。对比两组治疗的临床疗效,用EASI评估湿疹面积和皮损严重程度,对瘙痒症状改善情况进行评价,监测治疗前后外周血嗜酸性粒细胞（EOS）计数、血清白细胞介素-18（IL-18）、血清免疫球蛋白E（Ig E）水平。结果：治疗组有效率90.9%明显高于对照组的有效率72.7%（P〈0.05）;治疗组患者治疗后的EASI评分、瘙痒部位、瘙痒程度、瘙痒频率评分下降较对照组患者更为明显（P〈0.05）;血清中EOS、IL-18、Ig E下降较对照组患者更为明显（P〈0.05）。结论：加用火针治疗的慢性湿疹疗效显著,利于减轻湿疹面积及严重程度,改善瘙痒症状,其机制可能与降低血清中EOS、IL-18、Ig E水平有一定相关性,值得进行临床推广和进一步研究。Objective ：To investigate the clinical effect and the influence of EASI,pruritus score and serum factor levels for treating chronic eczema with fire needle. Methods ：66 cases with chronic eczema were randomly divided into a control group of 33 cases and a treatment group of 33 cases. Patients in the control group were treated with cetirizine hydrochloride tablets;while patients in the treatment group were treated plus with fire needle,one month for treatment. The research was focus on comparing the clinical effect of two groups,evaluating the eczema area and severity with EASI evaluation and the improvement situation of itching symptoms,and monitoring the peripheral blood eosinophils（EOS）count,serum interleukin-18（IL-18）and serum immunoglobulin E（Ig E）levels before and after treatment. Results ：The efficiency for patients in the treatment group was 90.9%,which was higher than that of the control group 72.7%（P〈0.05）;The EASI scores,itching position,itching degree and itching frequency scores of the treatment group after treatment were lower than that of the control group patients（P〈0.05）;the levels of EOS,IL-18 and Ig E in the treatment group were lower than that of the patients in the control group（P〈0.05）. Conclusion ：Treating chronic eczema with fire needle has good effect,it is good to relieve the eczema area and severity and improve the symptoms of itching. The mechanism may be associated with the regulation of EOS,IL-18 and Ig E,is worthy of further research and development.
