百合固金汤超声电导仪药物透入对复治肺结核患者血清免疫功能及炎症因子水平影响研究

摘　　要：目的：探讨百合固金汤超声电导仪药物透入对复治肺结核患者血清免疫功能及炎症因子水平的影响。方法：选取2013年11月—2016年4月收治的复治肺结核患者94例,按治疗方法的不同随机分为两组,分别为47例,对照组采用2HRZE/4HR标准化疗方案;在此基础上,研究组采用百合固金汤口服及超声电导仪药物透入治疗,持续治疗3个月。治疗前和治疗3个月后分别收集所有患者的血液标本,对炎症因子和免疫功能进行测定,对临床症状变化情况进行评分,同时比较两组间治疗效果及不良反应。结果：对照组改善率74.47%低于研究组改善率91.49%,有统计学差异（P〈0.05）;与治疗前比,两组治疗后IL-6、TNF-α、IFN-γ值降低,CD3^＋、CD4^＋、CD4^＋/CD8^＋值升高,CD8^＋值降低,咳嗽、咳痰、胸痛、发热、盗汗等症候积分降低（P〈0.05）;与对照组比,研究组治疗后IL-6、TNF-α、IFN-γ值较低,CD3^＋、CD4^＋、CD4^＋/CD8^＋值较高,CD8^＋值较低,咳嗽、咳痰、胸痛、发热、盗汗等症候积分较低（P〈0.05）;两组治疗期间血、尿、离子、肝肾及心电图无变化,无不良反应出现。结论：百合固金汤超声电导仪药物透入有利于提高复治肺结核患者的免疫功能和治疗效果,降低炎症因子,对肝、肾影响较小,是一种有效的治疗方案。Objective ：To investigate the influence of Baihe Gujin Decoction by electrophonophoresis drug penetration in the treatment of retreatment pulmonary tuberculosis（TB）in serum immune function and inflammatory factors. Methods ：94 cases of retreatment pulmonary tuberculosis were diagnosed and treated in our hospital from November 2013 to April 2016 were selected. They were randomly divided into 2 groups according to different treatment methods,47 cases respectively. The control group was treated with 2 HRZE/4 HR standard chemotherapy regimen,the study group received more with Baihe Gujin Decoction orally and ultrasound conductimeter drug penetration treatment,continuous treatment for 3 months. The inflammatory factors and immune functions were measured before and 3 months after treatment,and the clinical symptoms were evaluated,the therapeutic effects and adverse reactions between the two groups were compared. Results ：The improvement rate of the control group 74.47% was lower than the study group 91.49%,which was statistically significant（P〈0.05）. Compared with before treatment,the IL-6,TNF-α,IFN-γ decreased in 2 groups,levels of CD3^＋,CD4^＋ and CD4^＋/CD8^＋ increased,CD8^＋ value decreased,scores of cough,expectoration,chest pain,fever,night sweat and other symptoms decreased（P〈0.05）. Compared with the control group,levels of IL-6,TNF-α,IFN-γ were lower in the study group after treatment,CD3^＋,CD4^＋,CD4^＋/CD8^＋ value were higher,CD8^＋ value were lower,scores of symptoms of cough and expectoration,chest pain,fever,night sweats and other points were lower（P〈0.05）. There were no changes in blood/urine,ion,liver and kidney and ECG during the two groups,and no adverse reactions occurred. Conclusion ：Baihe Gujin Decoction by electrophonophoresis drug penetration is beneficial to improve the immune function and therapeutic effect of pulmonary tuberculosis patients,reduce the inflammatory factor of small liver/kidney effect,which is an effective treatment scheme.
